As Qaulity Assurance Coordinator (Civil/Structural) on a new Potash Mine development in southern

Saskatchewan my responsibilities involved the following:

• Establish and help maintain the Project Quality Management Plan onsite including written QA/QC

procedures/ Inspection Test Plans (ITPs) for all identified processes utilized by the site team

members, including the compliance of Codes/Standards, and standard deviations process

• Ensure all procedures and processes utilized by the site team were documented and included in the

QMS (Quality Management System)

• Ensure Non-Conformance Reports (NCR) and Corrective Action Reports (CAR) were efficiently

reported, resolutions adhered to and tight monitoring to ensure timely close-out of NCR and CAR

related issues.

• Ensure all subcontractors are approved for the project work. Participated in reviews of contractors

QA procedures, Weld Procedure Qualification (WPS)/ Performance Qualification Record (PQR),

Kiewit Western District

2010 to 2012

Civil & Earthworks Inspector Port Mann Highway 1 Project

QA/QC Earthworks Inspector on the Port Mann project responsible for earthworks and EPS

(Expanded Polystyrene Geofoam) construction along the 37km upgrade of Highway 1. Duties

included implementing and reviewing field technicians performance to ensure adherence to

industry technician specification standards (ASTM/AASHTO/ACI) and internal Quality

Management System performance.

Civil & Mechanical Inspector Kearl Oil Sands Project

During the first half of 2010 I was Lead Quality Inspector at Kiewit’s K2 Mining division involved in

construction of Canada’s largest open-pit mining operations ($5 to 8 Billion CDN). I was lead

reviewer and field inspector responsible for reviewing all soils testing, drilling data and compaction

reports completed during the Winter of 2009 to substantiate compliance to project compaction

requirements. During the spring of 2010, I was responsible for excavating and monitoring over 600

test pits. Reports submitted to the client included interpreted test results, survey data, maps of the

areas, and ultimately confirmed Kiewit had met or exceeded compaction requirements of the ETA

Dyke. I was also responsible for planning, organizing, and completing three each 100m x 20m test

strips of different zone material to meet compliance of project specifications. During the DeWind

Trencher operations I developed the field inspection logs

In the second half of 2010, I was transferred in August to Kiewit Energy, also on the Kearl site, where

I was assigned as Civil and Mechanical Inspector at the Froth Bitumen Plant.

Meadowbank Gold Project – 2008 (Spring) Baker Lake, NU

Qaulity assurance Civil Inspector during slurry wall construction of the East Dike using a soil-

bentonite technique. Responsibilities included precise monitoring of trench depths dug with a

hydraulic excavator prior to backfill using a mixuture of a soil-bentonite (SB) slurry pumped into the

trench after trench excavation. Quality Control activities were a prime responsibility and included a

fully functional field laboratory where primary soil index tests were accomplished under very strict

time constraints. Daily, weekly and monthly reports and test data were submitted submitted to client

and head office from field operations.

PKC Phase 5 Diavik Mine Construction – 2008 Lac de Gras, NWT

Provided geotechnical quality assurance and civil inspection for the 2008 Process Kimberlite

Containment (PKC) facility construction and other civil works construction at Diavik Diamond Mine.

Daily interaction with client and contractors to ensure compliance to project specifications .

winter 2008.

Stage VIII-B Tailings Dam Raise, Red Dog Mine - 2007 Arctic, Alaska

Lead quality assurance representative for the continuing raise of the tailings dam. Responsible for

field monitoring earthwork embankement and liner system installation. Equipment and record

keeping included extrusion welding, tensiometer testing, air pressure and vacuum testing.

Minto Mine - 2007 Minto Crossing, YK

Monitored curtain and blanket grouting of foundation and abutments to limit groundwater flow under

and through an earth fill dam as part of the mine development plan. Supervised selection of material

for dam core and outer shells. Conducted density tests using a nuclear densometer and proctor tests

in the laboratory.

Cape Dyer DEW Line Cleanup - 2007 Baffin Island, NU

Provided geotechnical and civil inspection services for 2007 DEW Line clean up project at Cape

Dyer. Services provided included borrow investigations, construction monitoring, as-built survey

review and materials laboratory testing, compaction testing using Troxler nuclear densometers and

field monitoring.

A418 Dike Diavik Mine – 2005 to 2006 Lac de Gras, NWT

Lead Field Representative for the A418 Dike Embankement Project. The project consisted of

permeability testing on plastic concrete, jet grout and curtain grout, including shear and stress/strain

testing. Grout testing alone consisted of monitoring the placement and obtaining over 3000 grout

cylinders and 1800 curtain grout cubes for compressive strengths. Other tests included mud balance

testing, filter press tests, Lombardi tests and Vicat tests, among others. Coring and sampling with a

variety of drilling equipment ranging from portable augers to truck and track-mounted drills. The

project was completed four weeks ahead of schedule. The engineering concept involved with the

A418 Dike was only the second time this method was used in the world.

Stage VII-B Tailings Dam Raise, Red Dog Mine - 2005 Arctic, Alaska

Senior field representative for the construction Quality Assurance Stage VII-B Tailings Dam Raise at

the Red Dog Mine, Alaska. Responsible for field monitoring of embankment earthworks, field

monitoring liner system (100 mill HDPE Smooth Liner) installation and raise and installation of

geosynthetics and concrete materials. Equipment and record keeping involved extrustion welder

(both non-destructive and destructive testing,) tensiometer testing and verification to specifications,

coupon cutter, air pressure tests.

Highway 99 Construction – 2009 Whistler, BC, Canada

Qaulity control civil inspector reporting to Prime Contractor and Ministry of Transportation.

Project included widening Highway 99 from Function Junction to Village Gate (7.8km) and

installation of BC Hydro utilities. In addition to monitoring the daily work of the

subcontractors and delivering daily reports, I was responsible for coordinating and reviewing

all soils testing, concrete tests and asphalt monitoring of testing crews in support of widening

the highway for the 2010 Olympics. Monitored all underground hydroelectric utilities, rock

blasting and asphalt paving for the Ministry of Transportation.

Concourse C Construction- 2002 to 2004 Anchorage, Alaska

Qaulity control representative during the 2-year construction of the new Concourse C terminal

at Ted Stevens International Airport in Anchorage, Alaska. Conducted sieve analyses,

Atterberg Limits, and Moisture-Density Relationship (Proctor) data on native material and

engineered fill. Fast turn-around of test data was critical as the engineering specifications were

only 60% complete at the start of the project .

Seward Highway Scenic Outlook Project - 2004 Anchorage, Alaska

Qaulity control representative on the Bird Creek to Girdwood, Alaska, Scenic Outlook Project.

Performed civil inspection and review of specification for rock wall construction monitoring on

four new Scenic Outlook areas. Reporting daily to State of Alaska Dept of Transportation

representatives.

Highway Rehabilitation Project - 2003 Alyeska Ski Resort, Girdwood, Alaska

Qaulity control representative to State of Alaska Department of Transportation on highway widening

project from Anchorage to Girdwood, Alaska (Alyeska Ski Resort). Monitored compacted soils prior

to asphalt placement and asphalt coring for payment.
