You will see my recent nursing experience has been Palliative, however I have broad experience in multiple/diverse nursing specialties: OR Acute Care Oncology Post op Surgical/Medical and Clinic care, Chart Review/Utilization Management including mock survey proceedings that required professional roles of autonomy/aptitude/critical thinking.
I was consistently successful at each role served. These areas of expertise include, but not limited to teaching of para~professionals, peers, medical/nursing and Graduate students, Management, Supervisory Leadership roles, to triage for acute and sub acute events, plastic surgery, internal medicine, assisted living evaluator and volunteer first responder participant.

3/09 to 10/12 Legacy Hospice ( start up Hospice ) in Charlottesville, Va…case mgr, mentor to UVA med students. Case load of 22 patients, fully responsible for pts’ and familys’ respect, symptom management, case audits and concurrent review on going assessment of medical/clinical choices

03/06- 3/09: Scott and White Hospice and Home Care Case Manager autonomous, uses outstanding critical thinking, calm in acute/emergent situations, personable, intelligent, well liked by colleagues, families and patients alike. Complete case management : fully satisfied patients and families. Medicare/Medicaid chart audits. Closely followed JACHO guidelines.
11/01 thru 2//06 : St. Francis Hospice Nuuanu/Pali, Honolulu, as Hospice RN Case Manager F/T census 13 patients:
Responsibilities include, but not limited to all Clinical Management of each patient with a life expectancy of less than six months. Teaching patients and Pts’signs and symptoms of dying with dignity, medications, interdisciplinary team members who contribute to patient and family support are respected and treated diligently.
8/2000-11/01 Lake Forest Place Illinois Clinic Manager and Hospice Liaison. Responsible for all aspects of Clinic Operations, in addition triaged all Emergencies for Independent Residents (150 Residents; staff of 112). Provided assessment of Hospice appropriate Residents responsible for recommendations Plans of Care to Senior Executive Management. Liaison Hospice RN with Lake Forest Hospice Personnel and clinics.
3/96- 7/00 Hospice RN Case Manager / Team Leader for Hospice of the Piedmont Charlottesville, Virginia. Designated RN to teach and train 4th year UVA Nursing Students, UVA Medical Students to satisfy their Palliative and Hospice Community Rotation requirements. Provided Community Hospice Lectures. Worked closely with Hospice Medical Director, In-patient Staff of UVA Cancer Center also Cancer Center at Martha Jefferson Hospital. In-patient Hospice Manager for new 6-bed Hospice and Palliative Care Unit at MJH. Trained new staff members. Worked closely daily with all aspects of Hospice Unit function, patients, families, Medical Directors, UVA Cancer Centre and Hospice Unit Team Members including Surgeons, Physicians from diverse specialties including but not limited to Board Certified Oncologists Charge Nurse Adhered to JCAHO standards of care criteria.
05/90-1/96 MJH Womans’ Surgical Evening Charge Nurse for 17 bed acute floor with occasional overflow of Mothers’ of fetal demise pts. Challenging clinical environment including significant provision of post-traumatic psychological care for pts, staff and family. JCAHO guidelines followed with consistent internal chart auditing and utilization review case management.
LPN3/81-11/86 Hospice and Oncology New York Presbyterian Hospital. Foundation to the Hospice Philosophy. Followed JCAHO guidelines. Chart audits/reviews. Utilization review/mgr participant.
