Develop tracking mechanisms to improve patient care provided by the heart failure clinic, Patient Aligned Care Team (PACT).

Plan, design and update various databases using Microsoft Access, SQL, ProClarity Analytics Software, SharePoint, and Microsoft

Excel. Evaluate departmental goals and operations to determine the data to be analyzed for process improvement. Using SQL

and ProClarity, create and modify database schema, query, report and form designs according to the needs of management.

Maintain, update and track all patient appointments for the clinic using Microsoft Outlook and VISTA. Assist in collecting and

Teleconferences (VTCs). Provided continual maintenance of two VTC suites equipped with a variety of audio/video, network and
