Caring for individuals with complex, severe and enduring mental health problems
in treatment and triage Mental Health settings.
Providing high standards of nursing care to them in accordance with up-to-date
evidence based professional practice and Trust policies, assessing of service
users' care needs creating their care plans, implementing them and evaluation.
Carrying out vital signs of patients and administering medication with permanent
staff as well as encouraging patients to take their medication.
I prompt patients to take their medication, take care of their personal hygiene,
facilitate community meetings, listen to patients concerns and present them to the
MDT.
Facilitating new admissions to the ward and transfers, orienting patients to the
ward environment, carrying out the property check to ensure the health and
safety of the patients and staff, carrying out their Initial Physical Health
Assessment as well as the holistic assessment of patients’ needs and compiling
their care plans.
Encouraging patients to get involved and participate in ward-based activities with
the occupational therapists, to avoid isolation and therefore, enabling and

Worked collaboratively with individuals to plan their care, facilitate their
recovery by encouraging them to participate in ward- based activities with the
occupational therapist and planning their care collaboratively.
Engaging with patients during community meetings on the ward or on a one to
one basis to discuss their concerns and their experiences with medication and
their side effects and encouraging them to comply with treatment.
During my placement I undertook the opportunity to take charge of a ward
shift several times and I have taken the lead in documenting up electronically
and handing over to the MDT, and have always been given feedback by my
mentors and the team.

Worked with the A and E Psychiatric Liaison Team where I acquired skills and
experience in carrying out patients’ Psycho-Social and risk Assessments,
demonstrated Safe and Effective Practice in Risk Assessments and
Management of vulnerable and acutely unwell patients with various mental
illnesses, planned their immediate care to meet their health and wellbeing,
and ensured their safety on discharge. I participated in carrying out
comprehensive Mental State Examination (MSE) on patients when they first
presented in A and E, and I gained experience in liaising with other agencies
such as the Community Mental Health Team (CMHT) and the Home
Treatment Team (HTT) by making referrals for further assessment of patients
presenting to A and E who were acutely unwell to ensure continuity of care.
Participated under supervision, in providing good and effective after care
advice to families of patients to minimise risks. For example, considered
contingency plans to care.
Dealt with complex cases, and despite however difficult they were, I still had a
duty of care.
Gordon Hospital (Gerrard ward) May-July 2012
Gained experience in facilitating admission of patients with various Mental
Health illnesses, dual diagnosis and challenging behaviours.
Facilitating the Assessments such as the risk and Initial Physical Health
Assessments as well as their Psychological, social and spiritual needs.
Experience taking charge of a shift and creating Care Plans collaboratively
with the patients to meet their health and wellbeing, participated in Care plan
auditing with my mentors.
Participated in ward rounds on a daily basis where patient’s mental state was
reviewed by the MDT to determine their care plan or discharge and handed
over to the team.
North Kensington (Home Treatment Team) February-April 2012
Worked with the ‘HTT’ where I participated in the Initial assessment of patients
who were being discharged from hospital wards or from ‘A and E’ and
admitted to the HTT.
Participated in reviewing patients’ progress and mental state in their own
homes or during home visits, followed them up and ensured they were taking
their medication or prompted them to do so.
Reviewed Patients’ Care plans hence facilitating their recovery.
Experience was also gained through participating in Triage meetings and
Care Programme Approach (CPA's) involving the MDT such as Psychologists,
Doctors, Consultant Psychiatrists and Community Psychiatrist Nurses (CPN)
to review patients' progress, determine their suitability for discharge to the
HTT, their mental state, medication and ensuring appropriate accommodation.
Experience was gained in liaising patients with other suitable agencies such
as social support groups to help them improve their health and social network
by engaging them in group activities to aid their recovery.

I greatly developed my communication skills as I had a great opportunity
engaging with patients with different Mental Health problems, challenging
behaviour or with dual diagnosis, and were learning to gain their
independence.
I was able to build constructive relationships with warmth and empathy using
good communication skills, participated in Patients’ Care Programme
Approach (CPA’s) discussing patient care holistically, their progress and
discharge, I understand Recovery model and its practical application and as a
trainee Nurse together with other qualified staff greatly aided the patients'
journey towards recovery.
