CLINICAL PHARMACIST;

Description;

• Identifies, resolves and presents potential and related problems by obtaining and evaluating

medical data including patient history, laboratory data and review patient profile for drug

interactions, allergies, and contraindications, adverse drug reactions to determine and

implement drug therapy.

• Communicates directly with physicians, nurses, and other health care providers to relate

information regarding patient drug therapy.

• Communicates and document drug information given to patients and or patient representatives.

• Supervise and monitors all patient drug therapy in designated area, for cost effectiveness and

communicates all possible cost saving recommendation to patients and physicians.

• Maintains and updates personal drug knowledge database to ensure accurate provision of

pharmaceutical care of general disease states as well as specialized knowledge in assigned area

of expertise.

• Participate in clinical research activities.

• Prepare and ensure imparting of training as an educator in departmental training program for

clinical pharmacy development as well as deliver presentation on regular basis to the pharmacy

and other health care staff.

• Maintain a proper documentation of all recommendations regarding drug usage and their

outcomes.

• Conduct drug utilization reviews and suggest strategies to overcome the deficiency found by

such reviews.

• Facilitate clinical pharmacist in investigating the adverse drug reaction reported to the

pharmacy department and compile a formal report at the end of each month.

• Plan and execute activities for rationalizing drug usage; monitor the efficacy of such programs

and suggest practical approach to improve the outcomes.

• Participate in educational activities; seminars and conferences.

• Participate in training and credentialing of staff for clinical activities.
