Supervising Quality control practices in various aspects of Microbiology laboratory. Participating in EQAS (External Quality assurance scheme). Guiding Intenal QC in Microbiology and Serology sections

Clinical Discussion with physician regarding empirical antibiotic therapy based on Preliminary Identification.

Alerting the consultants about the significant critical call outs in Microbiology

Notifying the Clinicians about the Seropositive cases, guiding the confirmatory tests & emphasizing the need for post-test counselling.

Clinical Interpretation, Authorization & Certifying all the Microbiology & Serology reports using the centralised laboratory information system.

Coordinating with the Clinical Consultants for the appropriate & specific tests to be done here/ outsourced for different infectious diseases.

Analysis of all the relevant positive cultures on day to day basis for-

1.Analysis of Hospital acquired infections including SSI,CAUTI, CLABSI and VAP

2.Intimation of all the notifiable infectious diseases to government zonal office through MRD

3.Microbiological /infection control inputs to all HCWs on various types of precautions based on type of infection

Formulating Antibiogram every 3 months once and sharing the data with stalkholders. Generating Antibiotic policy based on patient type & infection and guiding judicious use of antimicrobials using restricted antibiotic form

Promoting Antibiotic Stewardship through antibiotic working group and CME.

Co-ordinate with clinical & non-clinical departments regarding infection control & providing strategic guidance.

Supervise the daily & weekly maintenance of the Microbiology instruments & Serological equipments

Supervising resource consumption in the sections, analysing the requirements with weekly discussions

Addressing the various Pre-Analytical, Analytical & Post-Analytical issues in these sections as part of quality control

Attending to client (patients & attenders) queries & complaints pertaining to above sections.

Out of hours on- call service with regard to various queries by the HCWs & patients.

Reviewing and updating standard operating procedure for these sections on regular basis.

Continuous medical education for all Technicians for good laboratory practices through regular seminars, group discussion sessions.

Planning & implementation of appropriate measures for the maintenance of accreditation of the laboratory with national or international bodies- ISO, NABH, NABL.

Prevention of infectious hazards as a Lab safety officer

Instruct the HCWs with regard to appropriate Isolation Precautions (Airborne, Droplet & Contact) to be exercised when significant diseases are encountered.

Investigation & Management of the outbreaks

As an Infection control Consultant:

Clinical rounds along with infection control nurse in the hospital on a daily basis, interacting with the clinicians regarding the significant and critical test findings and providing them appropriate suggestions to assist in improved management of the patients.

Conducting Infection control committee meetings monthly basis with discussion on various aspects of infection control. Formulating infection control policy for the hospital and implementing guidelines. Ensuring the compliance.

Surveillance and analysis of Hospital acquired infections, Environmental threats in High risk areas, Device associated infections: VAP, CLABSI, CAUTI, SSI (using Prevention Bundle Checklists and audit Tools).

Teaching Doctors and Nursing staff – Microbiology/Infection control

Reporting environmental surveillance data from CSSD, Operation theatre, Burns ward, ICU, Endoscopy unit, Dialysis unit etc. Active education, training and supervision of members of the health care team on aspects related to infection control.

Monitoring of restricted antibiotic usage with coordination along with clinical pharmacist.Promoting Antibiotic Stewardship through antibiotic working group and CME.

Review and update hospital infection control policies and procedures contained in the infection control manual. Supervise Regular audits pertaining to Hand Hygiene, Bio-Medical Waste, Linen Management, Kitchen & Pantry area etc. Supervise the Vaccination pre-employment program, Vaccination & screening of food handlers. Policy on Post-exposure prophylaxis for an exposed HCW as per the exposure (Needle stick injury & other occupational exposure), significance of injury & immunity of the individual.

Instruct the HCWs with regard to appropriate Isolation Precautions (Airborne, Droplet & Contact) to be exercised when significant diseases are encountered. Investigation & Management of the outbreaks

Formulating and implementing the Antibiotic policy including surgical prophylaxis in the hospital, guiding the physicians and the surgeons towards minimal and rational usage of the antibiotics and thereby preventing the drug resistance.

I worked for 3yrs as Junior Resident in the Department of Microbiology, Pondicherry Institute of Medical Sciences (PIMS).

Job responsibilities included:

(1) Clinical microbiology services to tertiary care 550 bedded hospital

(2) Teaching MBBS and B.Sc. Nursing students

(3) Research.

Skills acquired during the training:

The training involved pre-analytical skills in specimen collection transport and accession.Analytical and post analytical skills were practiced through processing and interpretation of diagnostic tests. Skills in antibiotic testing, monitoring,treatment follow up and outcome were also learnt. Prevention of hospital infection through regular lecture and demonstrations to MBBS students and nursing staff. Skills learnt in in bacteriology including automated culture methods using Bact-T, anaerobic culture techniques, serology and immunology techniques including ELISA, immunofluorescence, immunochromatography and turbidometry methods. Mycobacteriology including molecular technique, culture and sensitivity, mycology and parasitology are the other skills which I have acquired.

Research Experience:

As a prerequisite for the award of MD degree, my dissertation work was on Detection of mechanisms of emerging antibiotic resistance, especially ESBL,AmpC β-lactamases, metallo-β-lactamases and carbapeneamses’.As a part of this I performed moleculartests such as DNA extraction, PCR and gel electrophoresis.

Providing high quality clinical microbiology services for standard of care to hospitals .

Integrating clinical microbiology with infectious diseases by interacting and coordinating with clinicians and other infectious disease specialists.

Providing impetus to hospital infection control practices by interacting with nursing and other paramedical staff and conducting targeted hospital surveillance

Providing input and facilitating the formation and adherence to antibiotic policies in the hospital by close interaction with the management, pharmacy and clinicians.

Research:

Special area of research: if opportunity arises I would like to work further on emerging antibiotic resistance and look for markers for early detection of drug resistance in clinical isolates.
