ADDENDUM TO THE RESUME OF TED KNIGHT

- Health Care Consulting Engagement Descriptions –

(Note: Ted Knight was responsible for all planning and proposal preparation, technical and fiscal

contract management, client satisfaction and reporting regarding all of the following engagements.)

INTERNATIONAL HEALTH CARE PROJECTS

Developed the INCE Salud Program for the Government of Venezuela. Established a program to train 26,000 medical auxiliaries in ten employment categories over a 30-month period. Also assisted in planning a training center in Caracas.

Evaluated the Costa Rican Ministry of Health, a managed care system, to help expand its organizational capability to manage 200 health facilities resulting from recent nationalization of private health facilities. The work included reviewing the existing organization and operations, realigning them to include the new facilities and establishing an operational work program to ensure internal modifications were made.

Prepared a strategic health plan for the Andean Region to incorporate improved health status with economic development activities. The plan set forth activities to strengthen health services in five countries, development of quality assurance center and establishment of regional training centers.

Evaluated the Ministry of Health of the Dominican Republic, a managed care system, to identify areas for infrastructure improvement. Conducted an organizational and operational review, identified departmental needs and prepared a two-year corrective implementation plan.

Evaluated the Ministry of Health in El Salvador, a managed care system, to identify areas for improvement in departmental operations. Created a technical assistance plan for implementing corrective activities.

Conducted a market study of the Caribbean Basin to identify opportunities for manufacturers of health products to establish offshore operations in Caribbean countries. The study determined the most suitable products, production requirements and country specific trade and business requirements. Developed operational profiles and contacted selected manufacturers.

Performed a feasibility study for acquiring St. Mark's Hospital. The work entailed conceptualizing a hospital-based managed care system. The engagement was performed in Singapore for a major hospital company to determine if the existing plant and service program could be profitably renovated and expanded to make acquisition attractive. The study required innovative research techniques due to the absence of comparative financial, service volume or epidemiological data. The study was favorable and the hospital was acquired.

Conducted planning seminars for the Clinica Anglo-Americana in Lima, Peru. The facility was in need of renovation or replacement. Conducted a series of interactive seminars primarily for the medical staff to help them identify major planning issues and considerations associated with deciding whether to renovate or replace the existing facility. The decision was made to replace the facility and a comprehensive infant health care center was developed.

DOMESTIC HEALTH CARE PROJECTS

Assisted the Capital Hospice in researching private foundation funding to support its programs and new research initiatives.

Assisted the United Medical Center in researching private foundation funding to develop a comprehensive primary care and health disparities center of excellence to serve the residents of Anacostia in Washington, DC.

Provided technical assistance to HMO demonstration grantees in New York in the areas of site planning, marketing, enrollee recruitment and service package definition.

Authored the HMO Feasibility Study Guide, which provided techniques and guidelines for conducting an HMO feasibility study under various organizational settings. The Guide identified the feasibility issues, their importance, data and analytical requirements and suggested feasibility criteria.

Authored the HMO Planning Guide to be used by demonstration grantees and technical assistance consultants to plan new HMOs and aid in their implementation.

Evaluated the 53 Indian Health System information systems for improving financial management. Reviewed the existing systems and identified alternatives for improving the value of those systems in Indian Health Service operations. Also developed personnel and inventory control systems. IHS is a managed health care system serving one percent of the US population.

Prepared cost finding techniques for the Alaska Area Native Health Service, a managed care system. Documented the existing health services and identified their cost of operations and procedures for cost allocation to obtain increased revenues from third-party payers such as Medicare and Medicaid and insurance sources.

Conducted a feasibility and role study of the IHS Mt. Edgecumbe Hospital and Service Unit for the Southeast Alaska Regional Health Corporation. The purpose of the study was to determine if renovation and modernization of the existing facility was justified to support continued inpatient, ambulatory and field health operations. Analyzed federal policy and future use of the hospital and service unit. Also identified the desires of the Southeast Alaska Native communities through interviews with knowledgeable individuals throughout the region.

Developed the IHS Health Needs Assessment Methodology to assist tribes in determining the health needs of their members and planning the appropriate health service responses. Documented procedures for setting programmatic goals and objectives, defining service requirements, modifying service programs and evaluating program implementation activities.

Evaluated and standardized the IHS Resource Allocation Criteria to provide a system that tribal planners could use in projecting manpower requirements at local health clinics and service units. The resulting documentation set forth criteria for each functional area and staff requirements at various projected workloads. Also prepared work sheets and instructions for step-by-step forecasting of health care demand.

Trained tribal personnel in applying the IHS Resource Allocation Criteria that were previously standardized for the Indian Health Service.

Researched and wrote the Quarters Leasing Manual for the IHS. The work was needed to help Indian Tribes develop quarters for federal health care personnel located on reservations and lease them back to the government. Analyzed pertinent legislation and policies, developed cost benefit analysis techniques and leasing arrangement options, identified potential funding sources and established the program guidelines for implementation.

Secured a $200,000 grant application for The National Patient Safety Foundation from the Agency for Healthcare Research and Quality. NPSA used the funding to develop a standard method of patient safety education to equip healthcare providers, administrators, and patients with practical, patient safety science knowledge to be applied to interactions with healthcare system. The education mechanism was a web-based basic and population-specific, patient safety continuing education with a supporting database on web-available patient safety resources.

Designed and developed the Infant Mortality Reduction Program to improve pregnancy outcomes in rural and urban areas. Prepared the IMR Program Operations Guide and Evaluation Workbook to assist community sponsors (primarily hospitals and health departments) implement the program.

Provided training and technical assistance to the IMR Program demonstration sites and evaluated their operations. Provided technical assistance to the 12 demonstration sites in their implementation activities, conducted regional site director meetings to communicate program findings and evaluated individual site performance and assisted in identifying additional funding sources to continue the work after the demonstration project ended.

Conducted a feasibility study for a new long-term care facility on the Navajo Reservation. The new facility was intended to serve tribal members with long-term care problems locally rather than receiving such care in facilities as far as 50 miles from their homes assuming space would be available in those facilities.

Prepared the Mauneluk Association Comprehensive Health Plan. Identified the service requirements and implementation activities, designed and utilized a planning methodology, collected and analyzed quantitative and qualitative data, and prepared the plan for this arctic region the size of the State of Ohio.

Conducted a feasibility study of the Ketchikan Indian Health Center. Evaluated existing services and assessed the feasibility of expanding service volume. Prepared a work program for Center personnel to expand their programs.

Assisted the White Mountain Apache Tribe in reorganizing its Community Health Representative Program. Analyzed existing organizational structure and position descriptions. Conducted an interactive seminar and developed a two-year operational plan.

Trained the Tribal Council and staff and prepared the Paiute Indian Tribe of Utah Comprehensive Health Plan. Researched and prepared the plan for the Tribe which consisted of five separate bands. Developed a "payer of last resort" system to maximize tribal IHS contract care funds. Negotiated with local health providers to ensure services were available to tribal members.

Trained the Yukon-Kuskokwim Health Board to improve operational capabilities and understanding of health care service delivery. The week-long seminar focused on group dynamics, decision-making, role of the health board, board/staff relations, operational planning, privatization of health services and parliamentary procedure.

Prepared and conducted seminars to train 12 Alaska Native Regional Health Corporations. The semi-nars addressed concepts of letters of credit financing, procedures for utilizing letters of credit and the effect of letters of credit on current contracting practices and procedures.

Provided organizational assistance to and trained the newly commissioned Program Formulation Branch of the Alaska Area Native Health Service. Conducted an operational analysis of the predecessor departments that were to be merged into the PFB. Reviewed AANHS mandates, policies and procedures. Conducted a two-day interactive seminar on management-by-objective techniques and developed a two-year work program for the new organization.

Conducted a series of training sessions for the Piscataway-Conoy Confederacy Tribal Council in planning, health care program design, health care financing, Council/staff relations, personnel policies and procedures and program evaluation.

Prepared a grant application for the Catawba Indian Nation to build a women's health center.

Developed the Comprehensive Elder Care Program Planning Methodology and applied it to the elder population of the Seneca Nation of Indians. The program included the feasibility of establishing a tribal nursing home and support services for non-institutional care of elders on two reservations.

Developed a marketing plan for a residential treatment center in Tidewater, Virginia. Analyzed the market potential and developed a market approach to attract emotionally disturbed children (between ages 6 and 8) to the center.

Served as an expert witness in health planning for Beverly Enterprises and several other nursing home companies in the Florida State Administrative Courts. Researched, developed and presented expert health need testimony in defense of new nursing home construction throughout the state.

Assisted in facility and certificate of need acquisitions for a newly organized nursing home company. Identified acquisition opportunities and provided pertinent background information.

Analyzed the Hill-Burton compliance regulations to identify barriers, which adversely affected free care pay back abilities of hospitals and other subject facilities. Surveyed key regional and state personnel to determine which regulations caused barriers and how they might be improved.

Developed the EMS Evaluation Workbook. Selected measurement criteria for evaluating system and patient process and outcome evaluations. The Workbook was utilized by grantees receiving federal financial support for emergency medical services to improve their operations.

Prepared the Emergency Medical Services Guide to Implementation for program managers to use in improving the capabilities of local EMS systems. Also developed a predictive model of organi-zational and environmental elements for federal personnel to use in determining the potential success of EMS grant applicants.

Developed the Health Planning Agency Governing Body Orientation Manual to assist agency boards in organizing effectively to meet the challenges of community health planning. Documented procedures for setting program goals and objectives, defining service requirements, modifying service programs and evaluating program implementation activities.

Prepared the Community Funds Flow Financial Data System Implementation Manual to measure the efficacy of health plans at the community level. The system measures the flow of health funds from sources to uses thereby establishing if the community plans are effectively altering the service delivery in a given community.

Provided training and technical assistance to the Experimental Health Care Delivery System Sites nation-wide in providing new approaches to health care delivery that would measurably improve the health status of the population.

Developed the St. Louis, Missouri Regional Acute Care Health Plan. Conducted health policy, service utilization, morbidity and mortality and alternative service modality analyses. Developed community goals and objectives and measurement criteria as well as specific recommendations for systems changes.

Prepared the Missouri Inpatient Health Care Policy Plan for coordination of future services and pro-grams. Defined the statewide policy base, interviewed knowledgeable individuals about state programs, prepared the policy plan and assisted state personnel in obtaining its acceptance by the governor's office.

Provided training and technical assistance to strengthen health planning agencies in fiscal management, operations, plan development and implementation, project review and analysis, data management, public involvement and policy definition. The entire engagement required the provision of over ten person-years of effort.

Conducted a feasibility study for the Kendall Lakes Community Hospital in Florida. The study involved preparation of the certificate of need application and supporting information, determining the community needs and range of health services and developing a preliminary master plan and a community wide marketing plan.
