DIETITIAN, Program of All-Inclusive Care for the Elderly (PACE) • July 2013 - Present The Jewish Home for the Aging’s Brandman Centers for Senior Care Part of IDT consisting of MD’s, NP, RNs, SWs, PT, OT, ST, Activities, Transportation. Attend daily IDT meetings. Perform full nutrition assessments on all participants and create care plan. Perform 6 month re-assessments with updated care plans and complete quarterly progress notes. Nutrition counseling with intensive meal planning and education (diabetes, renal, hypertension, heart disease, obesity). Create diet orders for participants based on medical needs. Coordinate home delivered meals and supplements. Monthly meeting with SLP to coordinate diets and treatment plans for dysphagia. Monitor monthly/weekly weights.
