Overall direction and supervision of medical model center whose mission is to enhance and prolong the quality of life for individuals who require supportive health care services to remain independently in the community. Plan, organize, staff, develop, evaluate, report, and budget as needs of program dictate. Supervise interdisciplinary staff. Ensure compliance with Adult Day Health Care Center rules, regulations and policies. Market the program both at the Medical Center and in the community at large.

Social Worker 2005-2012

Oversee, administer and coordinate delivery of services to registrants and their family/caregivers. Conduct six and

twelve month reviews, maintain appropriate documentation and monitor results for compliance with Center guidelines

policies and procedures. Screen, assess and evaluate Registrants prior to admission to ensure program eligibility.

Conduct detailed psycho-social assessment for new registrants, including counseling registrants and their families

/caregivers to ensure care coordination. Conduct outreach to community organizations, hospitals, agencies and other

resources for referral. Coordinate with community services not offered by ADHCC. Provide discharge planning,

follow-up and arrange transportation to ensure program continuity. Assess registrants for signs of mental illness

and/or dementia and make appropriate referrals.

Assist Program Director (act as program director during absences) and advocate for registrants.

Coordinate interdisciplinary review and monitoring and adjustment of each registrant’s care

Conduct staff meetings and in-service programs and serve as an active member of ADHCC interdisciplinary team
