CLINICAL RESEARCH EXPERIENCE

Adequate nutrition is essential in early childhood to ensure healthy growth, proper organ functioning, strong immune system, and neurological and cognitive development. Unfortunately Pakistan, a developing nation has a high infant mortality rate with malnutrition being one of the leading causes. Dietary counseling is an integral part of treating malnutrition. We assessed the nutritional counseling of children age less than 3 years by pediatricians at Jinnah Hospital Lahore during my 4th year of medical school. We observed th there were significant deficiencies in counseling of the patients and forwarded our recommendations to the hospital's administration. Those included creating a position of child nutritionist and arranging special classes for mothers on weaning off breastfeeding. My responsibilities were to gather data from the patient's parents, direct observation of the pediatricians and analyzing the dat Early insulin administration is associated with more rapid resolution of DKA without an increase in complication rates. The introduction of guidelines had moderately influenced the process of managing DKA but not the outcome, probably because of the low rate of their implementation. We observed instances at Carle Foundation Hospital of inefficiencies such as delay in initiation of insulin drip at time of DKA diagnosis, as well as an ineffective transition from intravenous insulin to subcutaneous insulin after the resolution of DKA. We were able to quantify this hypothesis into objective data and recognized that lack of implementation was significantly playing a role in increasing the length of hospitalization, mortality and patients satisfaction. So we implemented measures including changes in the order sets and physicians education. I had a leading role in the team of this project by writing the abstrac meeting with ED physicians, pharmacy and nursing staff, performing chart review and data analysis.

Sodium-glucose Cotransporter-2 Inhibitors [SGLT2i] and Glucagon-like peptid 1 (GLP-1) can reduce major cardiovascular events of MI and stroke in the diabetic population and are also associated with reducing hospitalization for heart failure and progression of CKD. We did retrospective chart review of diabetic patients that were seen in the resident clinic in last 3 years and recognized that therapeutic approaches used, were limited to metformin, sulfonylurea, and insulin. We identified and tried to reduce barriers for the decreased rate in the prescription of newer agents with residents teaching session and noticed a surge in the prescription of these agents afterward March 2014

Due to the intricate processes taking place during the development and progression of cancer, the tumor continuously interacting with its surrounding microenvironment and changes its dynamics. Specifically, in colon cancer, the fibrotic stromal response known as the desmoplastic reaction has been linked to survival, cancer aggressiveness and degree of invasion. Traditionally, after surgical resection/biopsy and molecular staining, pathological interpretation is used. We explored it via quantitative image analysis using infrared rays to evaluate the tissue biopsy images in the form of quantifiable variables. Part of my job during this research was to annotate the histological images of colon cancer slides and then afterward annotate the corresponding areas in the images obtained with infrared microscopy. If successful this technique can giv information on the histological characteristics, staging and prognosis of the cancer tissue during the colonoscopy even before obtaining the biopsy. Many things need to be followed post-hospital discharge and the patients should be seen in the outpatient setting within a crucial time period. This is quite significant for patients who are at increased risk of hospitalization especially heart failure, COPD and diabetes [coming in with DKA]. During the post-discharge follow-up visit, PCPs should be vigilant about hospital course and changes in medication list along with available support at home. We identified that Internal Medicine residents at Carle Foundation Hospital were straggling in some of the key components of hospital discharge follow up visits We did an educational session to maintain a checklist during these visits and addressed the key elements including reconciling the medication list, asking fo support available at home and ordering appropriate lab tests. The skills and knowledge gained from basic life support/advanced cardiac life support (BLS/ACLS) training is often volatile. A lack of hands-on experience can lead to anxiety and poor performance, especially by the residents. The analysis from surveying internal medicine residents showed that residents lagged confidence in running code blue/code 99. With the help of ICU attendings, we introduced mandatory simulation codes every week and also more autonomy was provided to the residents to supervise a real code blue in the presence of an ICU attending. After introducing these changes, we will do the post-intervention analysis in 6 months to quantify its effectiveness. Implant-based reconstruction remains the mainstay of post-mastectomy breas reconstruction. Over the past decade, a new paradigm shift has been made. Alloplastic engineered construct composed of an expander and fat grafting in the pre-pectoral space creating an aesthetically pleasing reconstructed breast with minimally invasive technique is the new approach. The primary objectives of our work are to identify any differences in clinical outcomes and complicatio rate between pre-pectoral and subpectoral reconstruction at a single institution by retrospectively reviewing the patient charts in the last four years. My role in this study is mainly doing extensive chart review and data entry into the data collection tool.

Rheumatoid arthritis (RA) patients have an increased burden of infections which in turn can increase the morbidity and mortality. This increased risk is n only a result of the autoimmune disease but is also attributed to the immunosuppressive therapies (e.g., glucocorticoids, DMARDs, biologics). Through extensive chart review, we identified that immunization rates of influenza, PCV 13, PPSV 23 and shingles vaccine were very poor in adult RA patients who were on immunosuppressive therapy. As an intervention, we hav planned to immunize patients for flu in the rheumatology clinic for the upcomin flu season. We also plan to introduce BPA alert for the rheumatologist to chec immunizations when thinking about prescribing these medications and send online communication to the patient’s PCP, informing them of the dire need fo these vaccinations. My role in this QI project was helping with chart review an analysis of the obtained data.
