3. May, 2012 to August, 2014: RESOURCE TEAM LEADER – Hospice by the Bay – www.hospicebythebay.org –HBTB is the 2nd oldest non- profit hospice in the nation.

Services are provided to an average daily census of 380 patients in Marin, San Francisco, N. San Mateo and Sonoma Counties. Reporting to the Director of Clinical Operations and serving as the After Hours Manager, my role is to oversee all nursing needs from 5 pm to 8.30 am during weekdays, and also weekends and holidays. Out of the 168 nursing hours in a week, my department covers128 hours of hospice care, with 20 full time nurses (13 of which are RNs; and 7 LVNS). There are also 15 non-benefitted per diem nurses and 10 CNAs reporting to me.

Supervise the planning, coordination, and delivery of the interdisciplinary team interventions;

Provide leader to the afterhours team in conjunction with the Director of Hospice Services to develop, review, and revise the patient/family plan of care, facilitate problem solving sessions, and delivery of team support;

Implement and monitor compliance with agency policies and procedures related to clinical services;

As a member of the Clinical Management Team, address problems associated with the delivery of clinical services and staff development needs, make recommendations for appropriate additions/changes in policies and procedures, and provide peer input to enhance and improve the middle management function;

Supervise the maintenance of clinical records ensuring accuracy, completeness and compliance with licensing regulations, certification standards, legal, and ethical standards;

4.February, 2011 to May, 2012 – DIRECTOR OF NURSING – Foulk Manor South –

Wilmington, Delaware FMS is a 133 semi-skilled bed facility. 75 of these beds were assisted living beds; the remainder were Medicare beds. We only accepted up to Stage 2 wounds with no skilled nursing care. I reported to the regional director of nursing, and my primary responsibility was to assist the Executive director in the daily operations of the facility. My team included 25 full time nurses and another 15 non-benefitted nurses. I also oversaw the nursing assistants and activities groups.

Develops and implements nursing service objectives, policies and procedures to maintain quality nursing care in the nursing home;

Determines staffing levels and monitor for adequacy in provision of nursing care;

5.February 2010 – May 2012 – WEEKEND ONCALL SUPERVISOR – Seasons Hospice Delaware - www. Seasons.org. Seasons Hospice is a for profit organization that serves three counties in Delaware. I reported to the Weekend Clinical Manager and was responsible for a weekend team of 15 nurses that covered nursing needs from Friday 5 pm to Monday 8am. Total census of 115 patients with diverse diagnoses, spanning three counties.

As a head nurse, I triaged the calls and ensured timely attendance to patients’ needs, delivery of medications, and effective transition of both patients’ and their families.

Attended weekly office meetings and participated in reports and communicated vital information back to the team;

Acted a go between the weekend staff who were primarily per diem and management who worked Mondays through Fridays.

Model a service attitude towards others, take ownership in solving problems and took the initiative to make things betters, being friendly and courteous to residents, patients and coworkers.

Attended and participated in orientation and appropriate in-services and staff meetings to improve knowledge and skills.

Provide appropriate support to dying patients and their families.

Ensure close communication among the patient, proxy decision maker, family and the IDT to ensure the best possible end-of-life care and honoring the patient’s preferences.

Regularly schedule care planning meetings that involve the patient if possible, the POA, family and IDT to facilitate communication about end-of-life care and help ensure that care is patient-centered and adapted to the patient’s changing condition.

Act as a guide during transition at the end of life. This includes teaching about the disease, teaching specific skills, and clarifying information and options. It also involves reflecting observations and assumptions about patient’s situations and care back to the patients. As a guide, I also act to sort out conflicting and confusing information and facilitate communication so that the patient and family are able to prioritize what they need and want to accomplish.

4. Feburary, 2006 To May 2012 (Part-Time Clinical Instructor/Nursing Educator) - Leads School Of Technology – This was a program designed for Licensed Practical Nurses. An average class was about 40 students in lecture and at least 8 students per each clinical rotation.
