Provide crisis psychiatric services to adult population in an inpatient crisis stabilization unit; provide detox services/care to persons desiring to medically detox from alcohol and drug abuse; rotate call with one other NP to cover for facility; admit new clients to facility, write admission orders; perform daily psychiatric assessments of clients and record findings in electronic medical record; make medication changes based on the needs of the client. I manage both medical problems and psychiatric problems of clients with the delegating protocol agreement of a medical physician as well as a psychiatrist.

Reason for leaving: to obtain more in-depth understanding of psychiatric/mental heatlh.

03/15-04/18 Nurse Practitioner Community Primary Care of Georgia ~ Acworth, GA

Provide care to post-acute care patients and residents in longterm care facilities; Post-acute care patients are individuals over 65 years of age, receiving Medicare benefits

and require therapy to recover from their surgery or illness which include disease processes such as pneumonia, acute exacerbation CHF, acute exacerbation COPD, general deconditioning, UTI, sepsis, respiratory failure, etc; long term care patients are individuals with debilitating illness that their families cannot manage at home such as dementia, CVA with paralysis, tracheostomy dependency, persistent vegetative state, quadriplegia; and others typically seen in the longterm care setting; my job responsibilities include performing acute care visits for patients having a change in status; performing monthly assessments on longterm care residents, performing new admission history and physical visits with patients newly admitted to facility; reviewing lab report, xray reports, CT scan, discharge notes from hospitals; evaluate dietary recommendations and pharmacy recommendations, talking with family members about their loved ones condition; Perform discharge education to patients and their family members when being discharged from facility; write prescriptions for medications to use at home: I have a DEA number to write prescriptions for controlled substances; I serve as an educator and mentoring resource for other nurse practitioners and nurses; I orient and train new graduate nurse practitioners coming to work for the company; I manage all the typical diseases that are seen in elderly patients residing at a longterm care facility to include HIV/AIDS and mental health problems such as schizophrenia, bipolar disorder, depression, anxiety, dementia, etc

longterm care; responsibilities include reviewing lab results to include PT/INR levels for Coumadin therapy; reviewing UA w/ C&S to determine if UTI is present;

reviewing results of xrays, ABI’s, CT’s, Doppler studies, etc; examining patients for acute problems and performing periodic exams per medicare guidelines; ordering medications, diagnostic studies; coordinating with other staff members to include social workers, nurses, physical therapy, psychiatrist to insure patient receives the highest quality of care we can provide. Began working with Dr. Patel on a part- time basis in Jan. 2012 and then became fulltime employee in June 2012.

Reason for leaving: Needed a job with insurance benefits, retirement, etc.

Provided primary care to adult dependent and military retirees; managed common acute and chronic illnesses; reviewed labs, xray results; ordered diagnostic studies and medications via computer; performed 100% computer charting; contacted patients via phone or letter regarding results.

Reason for leaving: Extreme computer problems; computers failing on almost daily basis; extremely high stress level job.

11/08-11/11 Nurse Practitioner

District Clinical Services / Columbus Health Department ~ Columbus, Georgia

Provide care to men and women infected with HIV and AIDS; provide screening tests such as TB skin tests, STD screenings, papsmears; order routine labs and review lab results; administer immunizations; monitor for opportunistic infections; perform physical exam and history on patients new to the program as well as those patients re- entering the program after being away for over 1 year; provide routine follow-up visits; provide care to patients needing acute visits for illness; collaborate with supervising physician to develop antiretroviral regiment; order medication refills; take patient telephone calls.

Reason for leaving; was not seeking new employment; was contacted by the owner of Potomac Health Care Solutions with wonderful offer; was hired without having face-to- face interview, phone interview only.

04/07-11/08 Nurse Practitioner

Outpatient Clinic / Columbus Regional Healthcare System ~ Columbus, Georgia

Functioned as a midlevel provider delivering care to an indigent patient population; patients range in age from 18 years to 65 years; provide care to patients having

Type II diabetes mellitus, hypertension, chronic obstructive pulmonary disease gastroesophageal reflux disease, rheumatoid arthritis, congestive heart failure, asthma, gynecologic problems, psoriasis, and other common chronic and acute illnesses; ordered labs, x-rays, CT scans, etc.; reviewed results; made referrals to other specialty providers; also provided care on mobile unit to homeless population; very high volume patient load and challenging position.

Reason for leaving: working mobile unit was high stress level; A/C and heat did not always work; as well as exhaust fumes caused me to have recurrent bronchitis; chose to leave due to health concerns.

07/06 – 04/07 Family Nurse Practitioner

Ellaville Primary Medicine Center ~ Ellaville, Georgia

Rural healthcare clinic providing primary care to a large Hispanic migrant farm worker population; treating same type problems as mentioned above.

Reason for leaving: clinic was owned by Sumter Regional Hospital which was destroyed by tornado on March 1, 2007 and clinic could not financially support two full-time midlevel providers.

07/05 – 07/06 Family Nurse Practitioner

Reason for leaving: very much enjoyed my job but wanted to expand beyond women’s healthcare.
