Responsible for managing patients with acute and/or chronic disease in the primary care setting

Work in collaboration with the entire healthcare team, implementing and documenting individualized plans of care that incorporate age specific considerations

Promote self-care and enlist patients and family in establishing realistic health goals and end of life care

Prescribe medications including controlled substances and adjunctive modalities per licensure

Obtains and documents relevant health and medical history including, physical examination based on history

Formulates the appropriate differential diagnosis based on history, physical and clinical findings

Establishes a plan of care to achieve the identified medical and health goals

Provide leadership for nursing and ancillary staff; acting as a clinical resource and role model

Act as a liaison as necessary between physicians, staff, patients, families and other departments to promote and facilitate communication, problem solving, decision making, crisis intervention, resource utilization and conflict management

Participate in nursing strategic initiatives such as quality improvement and monitoring, TJC compliance, Magnet Recognition research, recruitment and retention of staff and patient and employee satisfaction

Designation of daily shifts and monthly schedule assignments, ensuring adequate staff to meet current census and acuity of patients
