Responsibilities:

Determine participants' care by assessing program goals and needs.

Teach financial literacy to each participant. Monitor and document progress.

Establish and secure housing for each participant, i.e. Section 8, market rentals, Targeted housing etc.

Determine participants need for medical, psycho-social, and psychiatric evaluations after reviewing therapist

evaluations, treatment objectives, and plans.

Establish program goals by setting weekly or bi-weekly meetings to coordinate services being provided.

Arrange all community resources for each participant; including transportation and escort, if necessary.

Monitor cases by verifying clients' attendance, observe, evaluate and document commitment and responses.

Maintain financial goals for each participant i.e. credit reports, eliminating barriers to housing i.e. eviction debt.
