I want to provide a few recommendations. I love this template, but I would have liked to see narrative for some of the content as the narrative allows you to make connections with the information that was gathered. I do agree with your diagnosis and treatment plan. Wellbutrin XL is a great choice due to her symptoms of low energy, hypersomnia, and lack of motivation. I particularly appreciate the homework assignment you provided the patient.
