Durable Medical Equipment (DME) Lean Six Sigma Project.

Residency

California Correctional Health Care Services (CCHCS) is responsible for providing a community standard of health care to California’s inmate-patient population across 35 institutions statewide. At CDCR, the current process for Durable Medical Equipment (DME) procurement, replacement, distribution, tracking, and updates to various technological platforms is disjointed resulting in non- compliance in issuing DME. This problem exists state wide. This project aims to improve patient care by eliminating out of compliance DME issuance and free up wasted resources that result from the current DME process.

Involved in Lean Six Sigma project for timely issuance of durable medical equipment to increase patient safety.

Followed DMAIC methodology (Define, Measure, Analyse, Improve and Control) problem solving events to drive improvements.

Thorough understanding of all DME orders entered by Primary Care Provider (PCP).

Utilized Cerner Power Chart to extract DME order information details.

Conducted defect study for same day and 90-day DME issuance to the patient using the Cerner Power Chart.

Exported patient records data from Cerner prod to excel.

Completed Process Step Time Analysis to understand which process steps are consuming the largest amount of time.

Collected time stamp data on various steps of the order/procurement/delivery process.

Created queries to calculate the no. of days taken to issue DME to the inmate patient in excel.

Ensured the DME order process are in compliance with CCHCS policies and procedures.

Performed User Acceptance Testing for all the Cerner power forms which include Outpatient Nutrition Education in Sharp Healthcare.

DBMS (SQL) Term Project

Hospital Compare is a consumer-oriented database that provides information on the quality of care hospitals are providing to their patients. This information can help consumers make informed decisions about health care. Hospital Compare allows selecting multiple hospitals and directly compare performance measure information related to surgery, heart attack, heart failure, pneumonia, and other conditions. Hospital Compare is created by CMS in collaboration with organizations representing consumers, hospitals, doctors, employers, accrediting organizations, and other federal agencies. This paper reviewed claims data provided by the CMS (Centers for Medicare and Medicaid Services).

Created Hospital Compare database using a total of four tables.

Created Queries using Microsoft access to extract data from the access database.

Generated reports for top 10 Medicare spending hospitals, average score by score type, no. of hospitals from every state that reported complications and listed hospital name, city, state, score type and their corresponding score.
