Responsible for transitioning individuals from an acute care setting to a return to a home environment
Working closely with hospital case managers and medical teams in the collection of medical record data, assessment and interview of referred patients and family/caregivers, ascertaining physicians’ orders, educating the patient and family/caregivers of the delivery of home health services (with patient/responsible party input)
