Physician Assistant (APP)
Advanced Practice Provider (APP) in a new, expanding, fast-paced Neurocritical/Neurosurgical Intensive care unit at Eastern North Carolina’s largest Comprehensive Stroke Center. Involved in the collaborative care of critically ill and post-surgical patients, working directly alongside neurointensivists, stroke neurologists, neurosurgeons, fellow PA’s/NP’s, and critical care nurses. Following orientation, my responsibilities included the daily care of eight intensive level patients and up to six intermediate level patients, along with admissions, transfers, discharges, consultations, post-surgical follow-ups, and ED evaluations. Provided care for patients following Acute Ischemic Strokes, Intracranial and Subarachnoid Hemorrhages, Seizures and Status Epilepticus, Acute Encephalopathy, Hydrocephalus, Post-Cardiac Arrest Anoxic Brain Injury, Subdural Hematomas, Primary and Metastatic Brain Lesions, Acute Encephalitis, post tPA administration, and other neurological conditions. Managed post-operative patients following complex spine cases, decompressive hemicraniectomies, craniotomies and tumor resection, Management of additional acute illness and chronic comorbidities while in NSICU, including Elevated Intracranial Pressure, Malignant MCA Syndrome, Neurogenic Sympathetic Storming, Acute Respiratory Failure, Sepsis, ARDS, Aspiration Pneumonia, Renal Failure, Heart Failure, Hypertension, Atrial fibrillation, Diabetes, Hypertension, and COPD, and Cancer.
Applicable Skills, Duties, Responsibilities
Admitting patients to ICU from ED, general floor, PACU, and outside hospitals; writing initial comprehensive H&P’s, performing initial assessments, ordering and interpreting initial labs and diagnostic imaging, and formulating initial care plans with supervising intensivist and neurosurgeons.
Perform comprehensive stroke workups
Identify, assess, report, and treat acute changes in neurologic status
Provide stabilizing interventions and perform/assist bedside procedures for critical ill patients, including intubation, central venous lines (subclavian, femoral, jugular), and arterial lines (radial, femoral, axillary).
Leading and assisting in Cardiac Arrest Codes and Brain/Stroke code
Ongoing management of patients while in ICU; presenting patients on daily rounds, writing daily progress notes, monitoring serial lab trends, prescribing and dose adjusting medications, ventilator management, and treating subsequent conditions as they arise.
Consulting specialist services for input involving patient care
Peri-operative management of neurosurgical patients, including ordering pre-op labs and medications, troubleshooting and removing post-surgical JP Drains, EVD’s, staples and dressings, assessing patients post-operatively for pain management and resuscitation if necessary.
Transferring and Discharging patients to Intermediate Units, LTACHs, SNF, Rehab, and Home; writing notes, formulating plans, and arranging follow-ups for continued care.
Updating patients and families to ongoing care and prognosis, including goals of care, procedure consent, and Code Status discussions
Participating in continued education with ICU team, including lectures, Journal Club, and M&M.
Performing Death Exams and writing Death Certificates
