I have 2 years of experience in ADRdetection and reporting as a part of Pharm.D. I have spent 5th and 6th years of my Pharm.D. in Hospital for Clerkship and Internship.

Counselling and Patient Interaction:

Being part of Departments like General medicine (Male, Female), ICU, Gynecology, Pediatrics for 12 months as an InternI learned to counsel different kinds of patients regarding their medication. These sessions were completely supervised by Healthcare professionals. Counselling in Pediatric Department was mostly given to caretakers.

As a part of Pharm d 3rdyear I learnt handling of frogs and administration of drugs to animals.

Advanced learning Series-7(repeat) “ONCOLOGY” conducted at JSS hospital,Mysore, Karnataka.

Professional Experience:

KIMS HOSPITAL, Rajahmundry Mar 2017-May 2017

DESCRIPTION:

A six months hospital based prospective observational study was carried out in a NABH accredited hospital as a part of my Academic project. The study aims to find how an effective rehabilitation through follow up of about 3 months can effect the HRQOL of a ischemic stroke effected patients, as stroke is termed as a chronic disease with its manifestation across the life span and represents a major health and economic burden. The data was collected in specially designed data collection forms, from the case sheet of patients enrolled into the study.

RESPONSIBILITIES:

Educating the Ischemic stroke effected patients regarding complications of stroke and life style modifications to improve HRQOL of patients and to prevent recurrence of stroke.

Follow up of half of selected patients through telephone and half of selected patients on direct meetings for 3 months and comparing the effectiveness in 2 sets of patients.

CONCLUSION:

Our results highlights that in our patients,affected by Ischemic stroke, exits physical disability,severe thought disorders.The current study reported a significant impact of the clinical manifestation of Stroke, addressing life style modifications regarding diet and on the psychological status who were done follow up on regular basis than through telephone.So effective patient counseling by a clinical pharmacist and timely management is likely to prevent the patients from recurrence and fatility.

CLINICAL PHARMACIST INTERNSHIP:

RESPONSIBILITIES:

Internship activities include Ward Round Participation with Physicians, Patient Data Collection, Documentation, Prescription Monitoring, Identifying Drug Interactions, Adverse Drug Reactions and Patient Counselling.

Case Presentations, Journal and Article Reviewing with Seminars was conducted in Knowledge Sharing Sessions.

Follow Up of patients with prolonged hospitalization and Personalized Health Improvement Status was reported to Preceptor.
