My name is Rashidah M. Orr and I am applying for a position with your company. I have four years experience as a Behavioral Health Technician (BHT), previously working with SMI clients at Dessert Vista, The Psych Ward at Maricopa Integrated Health Services, Superstition Springs Mental Hospital and The Annex located in Phoenix Arizona.
While employed as a BHT, I continued my education at Arizona State University and May of 2008 I completed my Master of Social Work (MSW). As a student, my internship included working with SMI clients of all ages. My responsibilities included DSM IV diagnoses, Treatment Plans, Closures, Annuals, Crisis Intervention, Counseling, ASAM, Intakes and Assessments.
As an intern I have also had the opportunity to case manage clients for one year and six months with Child Protective Services in Phoenix Arizona. My responsibilities included a case load, referrals, court appearances, treatment plans, crisis intervention, home visits, supervision of family visitations, case documentation and closures.
I have experience as a Medical Social Worker. My experience includes MDS-Assessments, Quarterly and Care Plans, Behavioral Management, Risk Management, Psych, Counseling and Counseling Substance Abuse Special Focus Group.
I have worked as a Therapist completing in-home sessions with children and adults in the Foster Care System. These individuals were from abused, drugs, mental illness, and homeless experiences. Requirements included quarterly progress reports, assessment, treatment plans, documentation, attending mediation court meetings and writing reports for specialization determinations.
I was recently an employee of Corcoran State Prison as a Mental Health Clinician for the past 4 years. I have recently relocated and looking for employment.
I have had my certifications in CPR, First Aide, TCI and CPI. I am Trained in Restraints, Time Outs, One on One, Documentation, Vitals and Assistance with Daily Living Skills.
I am currently registered with the BBS as an ASW.
