8 Copy of Augmentation of Fire Fighting Tankers 2010 (Oil, Gas, Chemical).

9 Copy of degree / diploma / trade certificate (For Electrical Officer and ETO only)

10 Copy of Certificate of Competency (CoC).

11 Copy of CDC (All stamp pages)

12 Copy of Passport (1st and last page)

13 Copy of Indos Number

14 Copy of valid Medical Fitness Certificate in ILO form (DGS approved doctor)

15 Copy of updated DGS and MMD profiles.

16 Fees: Total Rs. 3050/- (Rs. 3000/- Certifcate + Bank Charges if any)
