Establishes patient’s goals and objectives that ensure high quality, cost-effective patient care, while working collaboratively with other interdisciplinary team

Provide care based on physical, psychological, educational, safety and related criteria for adolescent and adult population

Facilitates safe, high-quality, and cost-effective support upon discharge while working with interdisciplinary team as a discharge nurse.

Manages staffing, schedule, assignments, and patient flow as a charge nurse

Guideline-Driven medical therapies for chronic diseases to prevent readmission

Identify barriers for discharge
