Group home care for special needs individuals, making necessary appointments; maintaining quality health of individuals; monthly medication MAR updates and corrections; monitoring/maintaining of monthly logs for blood pressures, bowel movements, blood sugars, seizures, fluid logs/weight logs and reporting to physician; making/revising yearly special needs for individuals that include diagnosis, allergies, diets, risks, primary care physician and emergency information.
I also obtain physician orders and orders for adaptive aids such as wheelchairs, diabetic shoes, walkers, TED hose, splints, special eating utensils. I am tasked with teaching un-delegated staff how to do vital signs, medications and their purpose and how to properly handle them, how to read MAR, proper use of PEG tubes, foleys and nebulizer treatment machines; admission of new individuals, discharging and transfers of individuals; daily documentation and follow-ups; and act as the advocate for the individual. I provide wound care, PEG tube care, colostomy care, trach care, foley care/suprapubic care; passing of medications when needed. APS nursing assessments are completed as needed. I prepare pill planners when needed for family home visits. I act as the LVN on call for 7 days every 4 weeks or when needed.
On a daily basis, I assess individuals in need of urgent or ER care and prepare documentation for their appointments to include MAR reconciliation. I collect medical records of physician office visits, urgent care and ER notes, radiology reports, lab reports for medical charts and for PCP reference. I report relevant information to the RN.
