Position: Licensed Scheduler

Duties: - Assign and schedule homecare staff for customers based on demographics, customer needs and/or staff members skills.

-Readjust staff schedules daily for new admissions, customer issues, weather emergencies, etc.

-Assist with intake for potential new customers by gathering demographic information, verifying insurance, ICD 10 coding of diagnoses and accurately entering information into the EMR system.

-Gather information from the EMR for submission to insurance companies to obtain authorizations for continuation of care.

-Participate in the Quality Assurance program by gathering and tracking information in order to determine need for changes to policy/practices/procedures to insure compliance with government regulations and to further growth of the company.

-Serve as back-up for HR, Medical Records, Payroll.

-Coordinate communications between customers, clinicians and field staff.

-Assist Clinical Supervisors in obtaining medical information, orders, signatures from physicians to adhere to government timelines for continuation of care and completeness of the medical record.

Accomplishments: Given full control of staff scheduling after developing a system based on geographic location of client which decreased travel time between client visits and thus increased staff productivity.

Position: MDS Coordinator

Duties: - Maintain schedules for completion of both long term and PPS MDS's per government regulations.

- Consult with LTC MDS person to determine need for additional assessments for LTC clients.

- Schedule and direct the interdisciplinary meetings for both LTC and PPS clients and families.

- Coordinate daily with Therapy Manager regarding ARD selection to maximize reimbursement.

- Coordinate weekly Medicare meeting to determine continuation of care under Medicare guidelines.

- Complete nursing portion of all PPS and managed care MDS's within government regulations.

- Begin initial care plans for PPS and managed care clients.

- Monitor all other disciplines in completion of PPS and managed care MDS's.

- Educate and monitor staff regarding new electronic charting.

- Educate and monitor licensed staff in adherence to Medicare documentation guidelines.

- ICD-10 coding and sequencing for skilled residents.

Accomplishments: Increased Medicare reimbursement by $20,000.00 per month in the first 3 months after hire. Avoided expected temporary losses in Medicare reimbursement related to projected changes to the Medicare system by proactively seeking education regarding those changes prior to their taking effect.

February 2006 to June 2007 - East Longmeadow Skilled Nursing Center, East Longmeadow, MA

Position: Unit Manager Sub-acute Rehab Unit

Duties: - Monitor and coordinate ongoing care by licensed and certified staff on a 37 bed Sub acute unit.

- Develop and utilize various tracking tools to insure all documentation adheres to state regulations.

- Assist licensed staff as needed in areas of assessment, admission, supply, communication with interdisciplinary team and MD's.

Education: Valedictorian Dean Vocational Nursing Class. Completed all preliminary courses to enter the Associates Degree nursing program at Holyoke Community College but was unable to complete due to personal issues. Ended with a 4.0 GPA.
