Career Summary
Over the last three years, I have worked as a care manager, serving as a liaison between high risk patients with multiple complex conditions in an internal medicine clinic, in the state’s only academic medical center. In addition, I have been the main coordinator for a complex condition clinic known as High Impact Health, which focuses its efforts on reducing readmissions and empowers patients with the tools to manage chronic conditions at home. Furthermore, my initial nursing experience came from the Duke University emergency department. A busy and, at times, chaotic environment taught me how to prioritize, delegate and attend to the most pressing patient needs first. I have shown throughout my nursing career, the ability to adapt to varied environments and learn new skills through different presented challenges. I thrive in an environment of a fast-paced, challenging setting which fosters learning and collaboration, focused around patient-centered care.

oNurse leader liaison between the physician and patient in a team-based approach to care for individuals with complex health care needs. Designed and initiated a system of identification and follow up with each patient discharged from the inpatient and emergency department setting to ensure successful transition back to the outpatient setting. Accomplished effective patient education, self-management support, and timely health care delivery. Under my enrolled patient education program, there was an average 2.3% drop in A1C in diabetic 43 patients.
oSupervised and managed the High Impact Health clinic for highest risk patients and highest utilizers of inpatient and emergency services. Advocate within an interdisciplinary team of physicians, pharmacists, dieticians, social workers, nurses and integrated behavioral health which helped to decrease readmissions and lessen length of hospital stay. Reduced hospital readmission of enrolled patients by 43% after 6 months of intervention.
