Work from home on contractual bases for council on aging which contracts with Department of Community Health . This program manages clients at home or ALF to prevent institutionalization and to assist with aging in place. I am the admission RN Care Coordinator for 6 counties. I complete initial comprehensive in- home CCSP assessment using Harmony to determine eligibility for Medicaid waiver program. Data obtained is entered into computer and transmitted to GMCF for approval to program. Once approved, orders are entered, and services are brokered via SOWEGACOA.. I work with clients for approximately 6 weeks during determination /approval procees. I love this position, but company I am with did not receive up coming contract to manage program with DCH.
