Duties and Responsibilities:
Under the direct supervision of the Head Nurse. Acts as an advocate and health educator of patients and their families. Renders quality nursing care to patients. Maintains the availability of supplies and equipment. Maintains cleanliness and orderliness of the unit. Performs administrative functions and participates in the activities of the University, Hospital and Nursing Service Office.
1.Receives endorsement from the outgoing shift and make rounds. Endorses to the incoming shift.
2.Tenders quality nursing care to patients in the unit
a.Receives and orients newly admitted patients to the ward set- up;
b.Admits patients directly to the room of choice as deemed necessary or as instructed by the Attending Physician;
c.Takes, monitors and records vital signs of patients. Refer to the Physician any abnormalities;
d.Prepares and administers medications (oral, IM, IV, SQ) and treatments in accordance to the principles in drug administration’
e.Observes and assesses patients and records significant reaction to drugs and treatments. Informs the Physician and Head Nurse for any untoward conditions;
f.Answers and attends to patients’ needs and requests for additional help if needed;
g.Prepares equipment, supplies and instruments and assists in the procedure and examination to be done to the patient;
h.Make rounds as often as possible to know and check patients’ needs and condition; and
i.Records and documents pertinent observation, signs and symptoms and what was done and given to the patient in the patient’s chart.
3.Joins Physician’s rounds and keeps himself informed of special orders concerning the patient;
4.Carries out and/or transcribes Doctor’s orders in the kardex and chart. Does comprehensive chart rounds;
5.Signs discharge slip of patients who will go home; gives health care instructions regarding their medications, diet and activities;
6.Maintains adequate stocks of supplies and ensures availability of equipment for use
a.Checks that emergency medicines, stocks and supplies are complete in the crash cart. Sees to it that all medicines, supplies, articles in the same are replenished when used;
b.Checks that equipment are in good working condition. Informs the Nursing Office for job order of repair of equipment;
c.Responsible for requesting stocks/supplies in the absence of the Head Nurse; and
d.Makes necessary charges for articles, supplies, medicines and use of equipment.
7.Maintains cleanliness and orderliness in the unit. Checks that all rooms are cleaned and prepared for admissions. Informs the Housekeeping Aide for cleaning of rooms
8.Performs clerical and administrative functions
a.Request for laboratory and other diagnostic tests as per Doctor’s orders and follows up on results;
b.Accomplishes referral slips and forwards the same to the concerned department;
c.Prepares total census of patients every shift;
d.Prepares the diet list and informs the Dietary Department for any change in the diet of the patient;
e.Prepares and accomplishes transfer slip when needed;
f.Notifies the Hospital Chaplain of patients in need of his services (e.g. anointing of the sick); and
g.Supervises Nurse Trainees, Nurses’ Aide Affiliates and Student Nurses in the absence of the Head Nurse.
9.Adheres to Hospital Rules and Regulations and reports any variance from the Standard Operating Procedure;
10.Practices safety measures in the workplace and complies with the Hospital’s Infection Control Policies;
11.Renders assistance in other wards whenever necessary;
12.Participates in University, Hospital and Nursing Service Office activities;
13.Attends units, departmental and general meetings as scheduled by the Nursing Office;
14.Takes initiative in updating his own basic skills, knowledge and competence by attending in- service trainings, seminars and/or enrolling in the continuing education programs;
15.Recommends/suggests improvements on areas concerned; and
16.Performs such other duties and functions that are connected with, or in relation to, or arising from the performance of any of the above on his own initiative, and does such other duties as the Head Nurse may require him from time to time.
