An underserved, 340B, government clinic where pharmacists and prescribers work together to give the best care at a discounted price. Prescribers referred patients to the pharmacy so we could provide recommendation on diabetes, warfarin, hypertension, and breathing medications. I ran the visits alone to get the patient history, made medication recommendations to the preceptor, and counseled the patient on specific areas depending on what was discovered during the visit. I also “worked” patients up prior to visits, set appointments with patients for initial visits and follow-up appointments, finished patient SOAP notes, calibrated the spirometer weekly, collected data longitudinally for a government study, presented on updated guidelines, and filled prescriptions in the pharmacy

A high-volume community pharmacy striving for excellent services and patient safety. Daily tasks included reviewing patient’s chart to optimize medication therapy, discussing recommendations to the stroke team during rounds, and counsel patient on discharge medications. Other tasks included presenting lectures to P1-P3 students, weekly meetings with the preceptor to review concepts learned throughout the whole rotation, and researching clinical questions proposed by the stroke team to discuss potential resolutions.
