I performed patient follow ups and call backs as part of an 1115 B Waiver program monitoring patient blood pressures and medications. I recorded and generated reports for the 1115 B waiver program and reported to the Quality Assurance Officer and Chief Nursing Officer. In addition, I work with the case manager and serve as her back up when needed. I have held multiple roles in my career at Goodall Witcher Hospital. As Department Manager of Cardiac Rehabilitation, I monitored cardiac rhythms and supervised patients while exercising. I designed individualized care plans for patients to improve their cardiovascular fitness, overall health, and risk modifications. I have provided direct patient care in an acute care setting as well as supervision of direct patient care on a medical surgical unit with 25 beds, including 4 critical care/monitored beds.
