11/23 /2015 - 10/10/2017 Premier Family Medical - Clinics: Urgent Care - evenings

until 9:00 PM and Saturdays from 9:00 AM to 6:00 PM. Available

to walk-in Clients for fractures, lacerations, toenail removal, soft

tissue injury, acute illness, respiratory issues, GI issues, LABS,

etc. M.D. available by phone for consult (Jim Ngatuvai MD)

03/2010 to 2016 Onsite Medical Providers, LLC: Contracted with Doctors,

Clinics, Family Practices, Urgent Care Providers and others

Providing PA-C services. Including Home Health Care Corp.s

& Assisted Living Facilities. All services with oversight by a

Physician Medical Directors.

Dissolving LLC in 2016. LLC closed in 2017

12/2011 to 11/2014 Blue Rock Medical Center: Provided Urgent Care, After Hours,

Walk-in, and Family Practice services. Clinic was created by

Barry Clanton PA-C Manager and David Miller PA-C (myself)a

collaborative effort. We were contracted by the oversight M.D.

Radiologist to provide coverage and administrative workloads.

Later this went to part time applications under contact.

OVER

06/2007-03/2010 Internal Medicine Clinic (including “House Calls”): Provided

Clinical “Adult Medicine” and a broad spectrum of health care

services for complicated and interconnected chronic illnesses.

Workload required significant in-home care with oversight being

provided by a noted M.D. via mobile phone and as otherwise

required. Efforts were focused on flexible patient care and growth

of the doctor’s practice through expanded in-home service

Alternatives.

03/2006-06/2007 Aspen Ridge Rehab; South Valley Rehab; and Rock Canyon

Rehab: Services were contracted as a Registered Nurse / P.A-C.

Efforts were focused on Rehabilitative Medicine and Nursing.

Specific duties included: peg tube feedings, wound care, mobility

machines, intravenous therapies, subclavicular port therapies,

injection therapies, etc.

03/2005-08/2006 Concentra & Physician “House Calls”: Worked directly with

M.D. to support Industrial Medicine (at Concentra) while also collaborating to provide in-home care (i.e., “House Calls”) for numerous geriatric patients. The scope of this practice included a wide variety of internal medicine. Most of the local Hospice / Home Health services knew us for the services we offered.
