I have extensive experience with often underserved populations with unique challenges including HIV, Aids, LGBTQ, Incarceration and History of Incarceration, Substance use disorders, homelessness, poverty, cognitive impairment, domestic violence and Behavioral Health diagnoses. I also have extensive experience in infectious diseases with resulting monitoring of vaccination protocols, wound care and antibiotic therapies. Work Experience:

Massachusetts Behavioral Health Partnership (initially as subcontractor with McKesson ) at both the Boston and Danvers offices. Integrated Case Management for diverse member group with emphasis on Behavioral Health needs including Depression, Schizophrenia, Bipolar Disease and Substance Use Disorders, homelessness, addictions and frequent hospitalizations. Most members often having concurrent medical diagnosis including, Asthma, COPD, Diabetes, and Heart Disease. Using a variety of means (telephone, computer based supports and face to face support modalities ), Integrated Care Management services include assessment of needs, medication review, reconciliation and compliance, identifying barriers and developing member centered goals to maximize success in helping members to benefit the most form their health care. This includes harm reduction and utilization of the most cost effective methods to achieve these goals. 2014 to present.

Beth Israel Deaconess Medical Center as Infectious Disease Outpatient Clinic as clinic nurse. My responsibilities included, medication renewals and approvals. Extensive vaccinations for both health maintenance and travel requirements. PICC line assessment and maintenance, Antibiotic therapy coordination and laboratory surveillance. HIV and Hepatitis C treatment including extensive patient teaching, wound assessment and strategies for wound care and dressings, assisting with Bone marrow aspiration and spinal fluid retrieval and follow up care. I developed a surveillance program for monitoring laboratory results and medication use to coordinate with the on line medical records program. 2006-2013

North Eastern Correctional Center and MCI Concord which are both facilities of the State Prison System. At NECC provided nursing care for between 150-280 inmates who were preparing for release from prison and reintegration to society. Chronic disease management was the primary focus with medication, lab studies and intake and transfer assessments included. I was the principle provider with physician support for 6 hours/week, psychiatrist support for 1/day /week and therapist support for 16 hours /week, during that time I achieved the best small site award from CMS the nursing contractor. 1996-2006

Albany Medical Center Hospital based acute dialysis unit. Hemodialysis for acutely ill patients or patients in transition including pre transplant, failed or newly placed fistulas and grafts, emergency dialysis including trauma based and co-morbid disease. Responsibilities included management of the inpatient unit including preparation of dialysate for procedures, on call for emergency needs, I also developed a demonstration based orientation manual for nursing members who worked in acute and chronic units and the standard operating procedures resource for the unit. 1991-1996

American Red Cross whole blood and aphaeresis units. Collection of HLA matched platelets for local hospitals and for national distribution if needed with independent on call responsibilities for matched platelets and white cell donations. I also took part in a national bone marrow donation program recruiting donors. 1987-1991

More Distant work experience includes high risk labor and delivery including C sections, therapeutic terminations, stress and non- stress tests, Pitocin and prostaglandin inductions, fetal scalp monitoring and vaginal delivery following C- section births.
