Rebirth-Paraprofessional

Primary duties include providing my caseload with referrals to resources within the community for assistance with food, utility, health, or educational needs. I also assist my caseload by developing skills to help the individuals overcome barriers that are blocking many from growing; to help build support systems, to help develop functioning skills for daily living practices, to assist the individuals in living, learning, and working in the real world day - to - day in hopes that they can function as close to their normal as possible. Responsibilities with both agencies consisted of customer service with potential individuals for services, individuals who were already in service with us, and businesses from group homes and senior high rises who wanted to know more about our community based services. I also checked weekly eligibility for both agencies to confirm individuals in our services were eligible to continue to receive services; make new charts for individuals new to agencies; enter individuals into Sharenote computer database for billing input, note entry and goals input, direct-care staff set-up in Sharenote and submission to staff advising of use; HR chart making and responsible for obtaining required HR documents per DBHDD and CARF’s manual for each file yearly; notifying all direct-care staff before the 1st of each month that invoices are due for payment on initial assessments, re-assessments and WRAPS completed that were assigned by the Clinical Director; enter all assessments (initial or re-assessment into Sharenote) for billing and to support if direct-care staff receives payment; set up company emails for new direct-care staff; set up agency orientation where once a month all new hires meet with both Clinical Directors and receive extensive training on documentation training, agency’s mission statement, policy and procedure, and HR.

This position is responsible for conducting, outreach and marketing efforts to the general community to include area employers, businesses, corporations, local, state and federal agencies, churches, civic and social groups and area schools and colleges; developing and implementing marketing strategies, creating, developing and producing all marketing materials to include: newsletters, brochures, flyers, pamphlets, workbooks and all collateral used in the agency’s outreach and marketing efforts. Specific and extensive duties for the Lease-to-Purchase Program include:

§Develop service delivery mechanism for program participants

§Managing and coordinating for lease participants in the newly implemented Lease to Purchase Program

§Working closely with the Macon Middle Georgia Housing Counseling Center to provide joint service delivery to homebuyers.
