Responsibilities: As a QIDP I was responsible for direct supervision of house managers and direct support professionals to include training and developing/supervising habilitation services and the daily operation of residential homes for intellectually disabled people. My responsibilities spanned to monitoring of correct services and prior authorizations from DDA/DDS. I developed person centered goals for home and community, participated and lead ISP meetings centered around a Person Centered approach, completed timely quarterly reports in full to include nursing reports, completed Person Centered Tools upon intake and annually or as changes occurred for team updates. The coordination of service implementation was a responsibility to include but was not limited to appointments, therapy completion, work site reviews and connections to community resources.

As a QDDP with experience working with ICF/MR individuals both children and adults I developed measurable annual program goals, supervised day to day operations of the residential program which included individual program plans, behavior programs, ensured safety of residents, recommended programmatic procedures and policies for approval. Participated in the hiring process and training of assigned positions in accordance with company policy and assigned staff promoting that each individual was treated as a valued member of the community. As QDDP I assisted with coordinating admissions and discharges to the program in conjunction with specified social worker. Responsibilities: As a Program Manager I coordinated youth care support plans with treatment team members which included provision with Managed Care Organizations, state representatives and guardianship. Youth serviced varied in mental health diagnosis, substance abuse treatment and developmental disabilities. I directly provided support and training to foster families to supply the highest level of care for youth placed in their homes. Made community connections in areas of educational, therapeutic and recreational need and monitored progress as applicable. In this position I balanced a caseload varying in different surrounding counties and referrals from various sources, maintaining compliance with 15-20 individuals.

Set and enforced policies, resolved disputes, administer dietary programs, trained staff and ensured residents were properly cared for. Supervised personal care of all activities of daily living, kept medical records daily, assisted with transferring and transporting of individuals, assisted in individual self-medication goals and administering of medications prescribed. Supervision and record keeping of compliance with all safety guidelines, support plans and regulations in accordance with effective communication of supervisor.
