OBJECTIVE:

As a NP, my goal is to provide quality care to a panel of patients that are seeking care in primary care center or in their home. WORK HISTORY

5/10/16 to 12/31/2017: Take Care Clinic/Health Care Clinics @ Walgreens

As a NP, I provided convenient care at a Walgreens clinic, as well as some primary care.

5/18/15 to 12/31/15: Providers Direct, PPLC

As a NP, I provided primary care in private homes and assisted living facilities, as well as in the company clinic. I also focused on palliative care in the home for those patients not eligible for hospice or palliative care under their current medical insurance. In addition, I provided coordination of care between different services such as APS, insurance companies, and other outside agencies as necessary to assure home bound patients remain safe in their home situation.

2/6/12 to 5/8/15: CareMore Touch

As a NP, I provided care that focused on primary care management of care in their home or assisted living location. I evaluated and treated a panel of up to 90 patients each week to assess for changes in condition, new onset problems, prevention and early identification of new problems. I worked with family members locally and nationwide to reassure them on the well-being of their loved ones.

As a NP, I provided care and focus on primary care management of patients in their home or assisted living location. I worked part-time and had a panel of patients that I visited on a routine schedule to assess for changes in condition, new onset problems, prevention and early identification of new problems.

Primary Assignment: Working at CareMore, a Medicare advantage program, where we provided medicare physicals and referrals to health education, initiation of prevention testing and programs within the health plan based on the US Prevention Guidelines.

11/2007 to 12/26/2010: Minute Clinic, Inc.

A retail Clinic in CVS pharmacies across Tucson

Provided routine care in a retail setting including diagnosis and treatment of minor illnesses & injuries according to company guidelines. Also provided education and testing for chronic illness such as diabetes and asthma. I also served on the Guidelines Committee and was involved in the local marketing committee.

1/1/2009 to 7/3/2011: Correction Corporation of America

Contract work Nurse Practitioner, Saguaro Correctional Center in Eloy,AZ. I provided annual physicals, primary care and urgent care needs at a private male prison.

11/05/2007 to 1/30/2009: University Physicians Healthcare @ Kino

Director Urgent Care/Nurse Practitioner

Role: Administrative support, process improvement & Nurse Practitioner care at the Urgent Care. I did both the director and NP job from 11/07 until 6/08 and continued on per-diem until 1/09. I left because of the management style of the medical director. The hospital is teaching hospital with many financial problems and there was talk about elimination of the position. After I quit, the position was not refilled and the facility went to a resident model for the urgent care without use of the role of the nurse practitioner.

2/2006 to 10/27/2007: Corrections Corporation of America

Red Rock Correctional Center/Florence Correctional Center

Provide ongoing care for 1200 male inmates. Care includes chronic problems such as pain, Diabetes, hypertension, COPD, Asthma, Hyperlipidemia & urgent care problems including injuries including suturing, sprains, strains and a variety of emergency situations. 4/2006 to 11/2007: Tucson Medical Center

Nurse Practitioner: Nuclear Medicine Department

Monitor patients having Adenosine Stress Testing. This included assessing for appropriateness of patient, completion of test and intervening in emergency situations.

4/2007 to 12/2008: University of Phoenix Adjunct Faculty

Taught classes for new Nurse Practitioner students. I had taught 2 classes of the NP role & 1 Geriatric Adult & Nursing Research. I quit working because of illness. I assisted on a PRN basis for the physical assessment class for several years.

11/2000 to 12/2014: Unity Center for Health & Well Being at Unity of Tucson, Inc.

A small private practice in a church setting based on a Parish Nurse Model with a full clinic on site. Closed because of decreased time to dedicate to venture and eventual plans for retirement & traveling.

Using the Faith Community Nurse role, the church clinic provided primary care for those who had no insurance or had Medicare. Part of the role also includes coordinate education classes, an annual health fair, and other support groups as needed. Currently Certified as Faith Community Nurse, as of 6/2010, through Southern Indiana University.

6/2000 to 6/2006: Carondelet St. Mary’s Hospital ED Triage Coordinator/NP

Provided education, quality insurance, triage and improvement process for triage. Part of my role included being part of a team that provided strategic planning of new 35,000sq.ft. ED. I also presented at a local conference on the improvement process in the ED. I have had articles published in ENA Connection on the different aspects of triage in the ED.

6/1996 to 6/2000: Staff RN Emergency Room Carondelet St. Mary’s Hospital

Provided emergency nursing care at level II trauma center in a busy ED to a variety of patients. Assist with education and precept of new staff.

10/1993 to 6/1996: Clinical Supervisor Carondelet St. Mary’s Hospital

Assess staffing needs and provide direction to the staffing personnel in meeting those needs. Provide coaching and direction to the staff nurse during crisis situations. Directly supervised 25 staff members, but during the night responsible for clinical and management problems for the entire hospital.

3/1989-10/1993: Staff RN Emergency Department Carondelet St. Mary’s Hospital

Provided triage, critical care and med-surgical nursing to a variety of patients that presented to the ED. Acted as charge nurse and as preceptor to new nurses in the ER. Involved in a team that explored the problems with length of stay.
