Title: Clerk II/Scheduler Location: Downtown Los Angeles, Kaiser Permanente 07/2017-10/2017

Temporary position as coverage for permanent employees out on leave. Reviewed referrals for patients post-op or bed bound in need of home health. Worked closely with multiple outside agencies to place with home care nurses. Processed and closed referrals with assigned agencies after faxing patient’s health information. Created authorizations for completed referrals assigned to agencies. Scheduled urgent/critical referrals with KP nurse’s and physical therapists, worked closely with all KP nurses and therapists. Assisted agencies with updated referral orders for existing cases for patients, directed patients with information in regards to their open referrals and trained new temporary hires.

Temporary positon for a trial project that targeted patients who received pre-natal care with Molina insurance. Phoned California Molina patients post-delivery to schedule in-home appointments with Nurse Practitioners to receive their 6 week post-partum follow-up. Verified patient’s insurance prior to scheduling in-home appointments. Responsible to fill nurse’s schedule for 6 counties, and complete a 10 question survey per patient visit. Mailed notice letters to all unreachable patients by phone after 3 tries each. Directed any patient that missed the 6 week post-partum window to follow-up with PCP or OBGYN to receive their post and preventive care.

Provide customer service by answering phones, attending the patients in the facility and entering data on a daily basis. Operate office machines such as copy machine, fax machine and computers. Assist doctors with patient translations, assist in the chart room, assist in time keeping, assist in payroll, release of medical records and subpoenas, lab work processing and completion as well as vehicle maintenance and mileage record keeping. Daily data entries consist of doctor appointments, medical claims, prescriptions, patient information and financial information.
