Collaborated with county agencies, court, school officials, service providers, and other appropriate parties for successful clinical interventions. Provided necessary coordination and ongoing monitoring of the provision of waiver and non-waiver services through counseling sessions to assure services were provided to assist the client in remaining in their home. As Case Manager performed community outreach education to the homeless and at risk populations to reduce and prevent HIV+ increase and spread. Distributed condoms and provided HIV/AIDS education and presentations to groups, the homeless and other high risk populations, and to those already infected with HIV, provided testing for willing participants and provided referrals to community resources and readjustment counseling after notification to those who tested HIV+ to assist them with adequate performance of activities of daily living and live productive lives. Individual, group, and family counseling provided for HIV+ clients.
