Work History
Social Services Coordinator August 2016 – Current
Patient Care Coordination – Philadelphia, PA
Using the Waiver programs from the Office of Long-Term Living, I have the platform to help physically-disadvantaged Pennsylvanians translate their problems into needs and work to identify appropriate providers to accommodate those needs and enable the individual to have the maximum level of independence. Service plan development, specialized documentation, brokering resources, and the facilitation of communication between consumers and providers are vital to the successful delivery of this effort.
Applied Behavioral Analysis (ABA) Therapist August 2015 – June 2016
Arc of Lehigh & Northampton Counties – Philadelphia, PA
Under the supervision of a BCBA, I employed various speech and language therapies as well as behavior management techniques to help elementary- and middle school-aged students develop social and life skills. I also would collaborate with the teachers and families of these students to create an action plan which engaged the child in healthy response behavior. Case management, working with families and seeking supervision comprised some of the necessary skills I used to excel in this position.
Community Health Worker August 2013 – August 2015 Neighborhood Health Centers of Lehigh Valley – Allentown, PA
I helped clients develop their capacity and access to resources including health information, medical insurance, food, and housing. I also helped people understand their health condition(s) and develop strategies to improve their well-being. Building social capital to support healthier behaviors were also crucial to the work I did, as was brokering people to both formal and informal resources. I would also conduct mental status exams, complete PHQ9 forms, and provide informal counseling, support and follow-up regularly.
Support Specialist May 2012- August 2013
Alternatives Inc. – Phillipsburg, NJ
I evaluated the skills and needs of each individual receiving services, then made recommendations to the program manager. These services included training in social skills, personal care, community awareness, daily living, money management and other areas based on the status of each individual. Additionally, I would schedule appointments, provide necessary transportation, and of course ensure security and confidentiality of all records.
