JOB DESCRIPTION
Under the general direction of the Chief/ Head nurse. Performs overall registered nursing care, specifically to the renal patients. Maintains optimum standards of nursing care and practice through assessment, planning, implementation, and evaluation of dialysis patient care. Hospital: Laguna Doctors Hospital Inc., is a privately-owned secondary hospital with 106-bed capacity, located at Sta. Cruz, Laguna, Philippines. JOB SUMMARY
Ensures that a holistic care is being rendered to hemodialysis clients of the hospital. A Nephrology Nurse cannulates vascular access, to facillitate dialysis, we also do catheter care and emergency hemodialysis. Monitoring of vital signs, attending patient who experience complications, health teaching, and creating plan of long term carein an out-patient basis using nursing process, are some of my resposibilities as a Nephrology Nurse. Detailed Job Descriptions
1. Provide direct care to patients undergoing hemodialysis based on the rules and regulations establish by the Department of Health and approved company policies and procedures. 2. Check and prepares all materials needed and ensures it is available before the start of hemodialysis.
3. Carry out doctos order and ensures that dialysis is perform accordingly. 4. Assess patient using the nursing process before, during and after dialysis. 5. Assess, inspect, regulate and monitor all machine safety and after dialysis. 6. Prepare patients access using the required standard for arterio-venous fistula and external access preparation.
7. Communicate with the doctor regarding identified patient related problem and initiates intervention for the same.
8. Administer medication as ordered and properly document it on patient charts. 9. Perform, monitor and record pre, intra and post dialysis treatment vital signs. 10. Initiate and perform Cardiopulmonary resuscitation in the event of cardiac and pulmonary arrest and see to it that all activities are supervised, organized and documented. 11. Update patients’ records during and after every dialysis session. 12. Do the inventory of e-cart medicines and stocks at the end of every shift. 13. Protect the patients from accidents, hazards and potential infections. 14. Observe the line of communication and applies appropriate communication technique in reporting, recording and relaying information of data.
15. Provide information and health education to patients and significant others regarding plan of care. 16. Attend Emergency Hemodialysis, when necessary
