LVN Bee First Hospice 7/31/17 – 11/16/2016
Under R.N supervision, performed home visits to assess and carry out required nursing services to assigned patients under hospice care. Services included; Physical assessments, pain and symptom management, wound care, administer/reconcile and order medications, perform Foley catheter procedures as scheduled, educate patients and their families on chronic and life limiting illnesses and assist family throughout its expected course. Report patients’ status to Charge Nurse and or PCP that should require immediate or timely intervention. Attend IDT meetings with Physicians. Document on all assigned patients. Rotate on call shifts.
LVN Care Path Clinic at DHR 04/11/16-7/28/17
Triage patients with chronic and life limiting illnesses by performing vital signs, functional assessments and reconciling medications. In addition, I also facilitated Physicians as a Certified Spanish Interpreter during patient/family meetings. Discussions included patients’ current medical condition, DNR status, and advance directives should terminal disease take its expected course.
Call-in or fax medication orders for refills to pharmacies. Fax patient information to benefits processing center and follow up with pharmacists and insurance representatives for prior authorization approvals, or other formulary medication options.
Participate in daily morning meetings with Physicians and Providers to review new hospice and palliative care consultations for the day, and review ongoing discharge planning for inpatients who require hospice services either in their home or in a facility.

Perform physical assessments, wound care, reconcile and order medications as needed and replace Foley catheters as scheduled.
