Brief description of duties and responsibilities:

Assessment, Planning, implementation and evaluation of patient care. Worked collaboratively with staff within the department to obtain departmental objectives .Perform admission and discharge procedures. Handle four patients in one shift. Monitor vital signs and calculate ADDS score to identify any clinical deterioration in client condition. Maintain clinical care plan for stroke patients. Perform neurological and neuro – vascular observation as per requirements. Perform procedures like ECG, over bed hoist as well as standing hoist and wound care according to the hospital policy. Perform bladder scan to assess residual volume in chronic renal patients.

Educate the patients and the families about self-care and available support systems. Care of patients with infectious diseases. Implement fall prevention strategies for elderly and unstable clients following fall risk assessment and management plan. Medication administration via oral, IM and sub - cutaneous including schedule 8 and schedule 4 drugs as per the policies. Administration of IV fluids and Blood products. Hypoglycemia management in diabetic patients. Act as an advocate for patients. Assist patients in activities of daily living and total care.
