PROFESSIONAL EXPERIENCE AND SIGNIFICANT ACHIEVEMENTS Commissioning PTW Manager - Syrah Resources Balama Mozambique 2017 – current Syrah Resources is an Australian resource company that is rapidly progressing its flagship Balama Project in Mozambique to production. The Project hosts the largest graphite ore reserves in the world. The processing plant will have a feed rate of 2Mt per annum, and based on an average head grade of approximately 19% TGC over the first 10 years of operations, 356,000 tons of graphite concentrate will be produced per annum.

- Responsible for coordinating with construction and safety to write and issue permits on site including but not limited to General safety work permits, CSE permits (reclassified and permit required), Excavation permits, NDT permits, night shift work, roof work and any and all special case activity permits. The Commissioning PTW Manager interacts with the existing LOTO team and report to the Commissioning Manager, with close coordination with the Commissioning Manager / Commissioning Superintendents. Interpret mechanical, P&IDs, electrical and plumbing drawings as well as project schedule.

- The Commissioning PTW Manager will ensure that the project LOTO procedures (mechanical / electrical / instrumentation) are implemented including appropriate resources for the work, organization of facilities, training, logs and monitoring in field.

- Commissioning carry out isolation from its energy source(s) preventing the release of potentially hazardous energy while Commissioning, maintenance and servicing activities are being performed. Manage the LOTO officers and ensure that planned isolations (as defined in IDC’s) are implemented accordingly. Provide feedback to management on implementation of project procedures as well as compliance. Carry out regular reviews on isolation plans and provide feedback to isolating authorities in case concerns are identified. Manage resources for LOTO and advise management on any possible gaps. Carry out regular audits and reviews on the process in place.

- Help Ensure SIMOPS are properly supported through use of the PTW system Dep/HSSE Manager/ERC – Worley Parsons Saudi Arabia 2014 – 2017 Serve as a Construction and Site HSSE Manager for Worley Parsons Arabia on this EPCM, Combined Cycle Power Plant Project: PP-13 with associated 380 kV GIS Substations and 380 kV Transmission Lines to connect to the existing SEC 380 kV Electrical Transmission Grid and interface with SEC’s SCADA system. Each power plant will consist of two blocks, with each block being a three-on-one combined cycle power block. Each power block will include three Gas Turbine Generators, three Heat Recovery Steam Generators, one Steam Turbine Generator and one Air Cooled Condenser. The power plant will be capable of generating 1,650 to 1,980 MW. Total project value: US$2 billion

- Managed, develop and implement HSE Site Management/R3 Plan; namely policies, guidelines, procedures as well as establishing the Permit to Work System and manage and train relevant personnel. Contribute to Project Execution Plan:

- Executed HSE Bridging document.

- Facilitated Project Base risk assessment.

- Facilitated Project Land Transport risk assessment.

- Developed Project HSE budget

- Ensure R3 standards, plans and procedures are in implemented and adhered to by WP as well as Contractors as per PEP. Conduct regular gap analyses against WP/Client standards with regards to site evacuations and Medical facility standards. Manage the WP Journey Management program

- Provide support on the implementation of companywide HSE initiatives as well as Improvement Programs according to OHSAS 14001 and OHSAS 18001 standards CV – Gerhard D Swart

- Develop, assist and implement HSE Communication structures and practices via internet, web portals and weekly/monthly campaigns to share know how and good practices

- Initiate, coordinate and perform HSE internal/external audits according OHSAS 18001 standards and analyses the results to prepare and improve current development programs/initiatives. Ensure regular inspections of tools, equipment, and vehicles are performed, deficiencies corrected and record maintained using appropriate forms

- Develop, assists and participates in the development of materials necessary for a comprehensive employee and supervisor safety training program, including training modules designed to meet or exceed the safety goals of the organization

- Identifies, investigates, reports and provides guidance, relative to root causes for incidents, employee safety programs, regulatory compliance also relative to safety and health and prepare/submit reports/findings and lessons learned. Ensured incident and accident investigations are conducted in line with Project and Client procedures and requirements

- Manage safety statistics, Behavior Based Safety Programs and improvement plans within Worley Parsons. Track and provide regular HSE performance reporting and indicators according to the Take Time For Safety Program to internal and external parties involved in the Project

- Perform / facilitate preparation, review and modifications of risk assessments as required; for all project site activities, facilities including camp accommodation and transportation

- Participates and assists in the development of health and safety policies, practices and procedures throughout the company in compliance with Local and International laws and regulations. Developed security organization and ensure adequate resources available.

- Ensure R3 Emergency Response preparedness provisions / plans in place and site employees are aware about the same. Ensure adequacy and availability of fire detection and firefighting equipment and all is in working condition for all site areas

- Prepare and submit necessary reports to Project HSE Manager / Site Manager / Project Manager/Director and Corporate HSE Director

Provide consulting service to ensure compliance to all relevant safety, quality and environmental management procedures and controls across the plant to guarantee employee safety, legislative compliance, delivery of high quality products/service and a responsible environmental attitude. Developing and establishing the emergency response plan for major accidents and provide the training.
