I have 18 plus years as an Occupational Therapist. Mainly, in Home Health Care, but have served in other roles as well; namely, rehab manager in a long term care rehab facility, case manager, mentor for COTA’s and rehabilitation specialist in human services and Memory Care with mental health individuals.

In my experiences I have evaluated patients, developed treatment plans, carried out treatments, documented on treatment visits and discharges, attended case conference discussions on patients progress, scheduled patient visits with other disciplines and supervised COTA’s, and coordinated care with facility based case managers and physicians. I am familiar with OASIS re-certifications and discharge procedures and have a working knowledge of HCHB and Rehab Optima for documentation.

CAREER SUMMARY

During my career as an Occupational Therapist, I have worked and provided care in several capacities. Most of my career has been in the home health care area as a home care therapist providing assessment of functional abilities, home safety activity, ADL functional abilities of assigned clients. I have served in management situations as administrator of an outpatient therapy clinic and Rehabilitation Director in Skilled Nursing Facilities. Also, as a licensed emergency medical technician, I have worked PRN status for EMS companies. I am currently certified as a CPR/AED/First Aid instructor.

PROFESSIONAL EXPERIENCE

-Complete evaluations with assigned patients in a variety of settings
