Duties include:

• Taking incoming calls from attorneys, state and private mental health evaluators, courts, transport teams, etc., to schedule attorney telephone calls, state evaluator video conferences, telephone and video court hearings, in-office record reviews, and in-person patient interviews

• Creating annual Chapter 980.04 and 980.07 Consent forms, Treatment Progress Report Consent Forms, and Requests for Counsel and/or Private Examiner forms

• Requesting all Court Orders from the Prosecution, the Defense, and/or the County Clerk of Courts and processing said in-coming Court Orders by notifying all applicable internal and external parties and recording all relevant legal status changes

• Coordinating and processing the dissemination of patient information from SRSTC to courts, attorneys, law enforcement agencies, the WI Department of Justice, county District Attorneys’ offices, and other requesters, and performing technical treatment records functions by maintaining active and inactive patient permanent records via both hard copy and electronic means

• Interpreting the federal and state laws in response to requests for patient information and informing internal and external parties if an information release conflicts with these laws

• Reviewing authorizations for release of information to ensure inclusion of all elements required by Wisconsin State Statutes and Administrative Codes, obtaining corrected authorizations if applicable, and meeting directly with patients to obtain signatures for confidential releases as needed

• After obtaining the appropriate authorization, preparing and complete and accurate copy of the record, composing both a documentation and notary letter and arranging for either electronic or hard-copy delivery

• Maintaining correspondence logs for the disclosure of all Protected Health Information (PHI)

• Working directly with patients in completing notary services and requests for information and other random services

• Daily data entry into several databases; maintaining master patient index and patient unit assignment/location system

• Preparing for and processing patient admissions, discharges, dismissals, transfers to Supervised Release, and deaths. Upon admission, meeting directly with patients to review/explain the Records Department admission forms and obtain signatures

• Receiving, reviewing, and filing patient information in the proper areas of both the treatment and medical records; maintaining records in an orderly manner and spot-checking for misfiled materials by performing routine audits; thinning treatment and medical records in accordance with both record retention laws and facility policy

Duties included:

• Investigating Unemployment Insurance benefits eligibility issues raised by employers, the actions/reporting of the claimant, the DWD, or other parties, which included:

Receiving a computer based claim (containing both data and documents,) identifying eligibility issues and analyzing said claim from every angle in order to determine an investigatory plan and an initial line of exculpatory questions

Obtaining background and supporting facts from employers, other parties, documents and DWD records prior to conducting a fact-finding claimant interview

Interviewing claimants via telephone or letter, eliciting claimant’s position/rebuttal in response to allegations that would suspend/disqualify them from the receipt of Unemployment Insurance, by asking background, probing, and clarifying questions

Preparing a clear and concise electronic statement from all parties, thereby documenting all information obtained

Analyzing said statements to identify and obtain additional facts/statements from either the principals or other parties if needed to resolve all eligibility issues

Evaluating Labor Market information through the Conditions of Employment Database to identify additional facts impacting the claimant’s benefit eligibility

• Making “Findings of Facts” necessary to resolve issues pursuant to requirements of the Department of Labor, Wisconsin Unemployment Insurance Law, Wisconsin Administrative Code, and other similar federal/state Unemployment Insurance laws;

Applying all appropriate laws, codes, and policies to prepare an electronic eligibility Rationale, determining requalifying wage amount if applicable

Preparing and issuing an initial eligibility determination, resolving eligibility issues in a succinct manner by providing all parties with the critical and decisive facts on which either party can base a decision to appeal (always ensuring prior to mailing the determination was factually correct, the findings were adequately documented, the decision reflected a resolution consistent with Agency law, and was written in a professional manner)

Entering the variable data elements resulting from the determination into the internal electronic database

Identifying potential fraudulent reporting (Program Integrity issues) and initiating a subsequent investigation and resolving said issues if necessary

• Assessing the claimant’s continuing eligibility for benefits; explaining the effects of various computations, initial determinations and appellate level decisions and providing general information about Wisconsin’s Unemployment Insurance Law and similar federal/state Unemployment Insurance Laws
