OBJECTIVE

To acquire a clinical position in a rehab facility or home care setting emphasizing Orthopedics, Cardiac, Pulmonary, Musculoskeletal and Neurological conditions, while continuing to develop interest in geriatrics.

Rebekah Rehab and Extended care, Bronx NY- 224 beds, sub-acute care facility- 2000-present.

Physical therapist supervisor with responsibilities to supervise staff in implementing treatment techniques and modifying plan of care according to the resident’s current level of function. Direct involvement in setting up treatment plan and goals, resident and family training, care plan meetings and “fall team” meetings. Evaluate residents on admission and design individual POC for their specific needs, perform quarterly and annual assessments including daily screening to determine the need for rehab services, conduct nursing inservice to ensure safety and continued care. Takes over responsibilities of the Rehab Director in his absence which includes completing MDS 3.0, attending D/C planning, establish appropriate RUG level under Medicare, Casamba planning, scheduling and documentations.

Fox Rehab, Weschester NY- house calls services- 2011-2014

Senior physical therapist rendering services in an independent living, assisted living and home setting. Independently responsible for evaluating patient using Raintree documentation under Medicare part B and design individualized program based on their Functional Outcome Measurement results, prior level of function and current level of function. Coordinate with the MD and RN on functional updates, current POC and goals. Direct involvement with patient’s POA, family and CG to ensure safety and carryover of daily HEP.

Mount Sinai Hospital, Manhattan NY- acute care in-patient setting- 2000-2006

Senior physical therapist acting independently in prioritizing patient care in cardiac units, conduct evaluation and treatment one day post surgery, communicate directly with the cardiologists, orthopedic surgeons, pulmonologists, vascular surgeons and other physicians. Update nurses on patient’s current level of activities, collaborate with social worker, nurse supervisor, rehab team regarding D/C recommendations.
