AIM

This paper is a report of a review conducted to identify and critically evaluate research investigating nursing practice in detecting and managing deteriorating general ward patients

SUMMARY OF THE BACKGROUND

Failure to recognize or act on deterioration of general ward patients has resulted in the implementation of early warning scoring systems and critical care outreach teams. The evidence of effectiveness of these systems is unclear. Possible mechanisms for low effectiveness may be inconsistent recording of patient observations by ward staff, or inconsistent application of 'calling criteria' for outreach teams, even when observations have been records.

ROLE AND RESPONSIBILITIES:

When a patient has suffered a heart attack, stroke, shock, severe trauma, respiratory distress or other severe medical issue, it is vital that they receive immediate and intensive nursing care. Critical care nurses are adept at providing such care in settings where patients can be given complex assessments and treatment.

Assessing a patient’s condition and planning and implementing patient care plans

Treating wounds and providing advanced life support

Assisting physicians in performing procedures

Ensuring that ventilators, monitors and other types of medical equipment function properly

Administering intravenous fluids and medications

Ordering diagnostic tests

Collaborating with fellow members of the critical care team

Responding to life-saving situations, using nursing standards and protocols for treatment

Providing education and support to patient families

Critical care nurses may also care for pre- and post-operative patients. In addition, some serve as case managers and policy makers, while others perform administrative duties.

Assist and support all staff and function in a team to provide care to residents at all times

Help to shower, bathe and shave patients while supervised by a registered nurse

Observe and report any changes in the client's condition or any complaints.
