JOB DESCRIPTION:

Assess, plans, implement and evaluate individual patient care according to standards.

DUTIES AND RESPONSIBILITIES:

Completes assessment of patient and documents in medical record.

Analyzes the information collected to determine patient’s needs and expected outcomes.

Observes, evaluates and reports symptoms and conditions of patients.

Develops a plan of care based upon the assessment, diagnosis, and expected outcomes of patients.

Administration of medications, monitor patients' response to medication and documents.

DUTIES AND RESPONSIBILITIES:

• Obtains nursing history for patients in the OR and DR.

• Performs physical assessment, takes and records vital signs once the patient is admitted.

• Identifies subjective and objective problems of the patients.

• Takes note of the abnormalities from the assessments and results of the diagnostic examinations and reports.

• Prepares surgical/OB instruments, equipment and supplies to be used during each operation or delivery (at least 30 minutes before the scheduled procedure).

• Ensures that all equipment/ machine are functional before each delivery or operation.

• Prepares patients for operations/diagnostic procedures and deliveries and assists the Surgeon/ OB-Gyne Consultants either as Scrub or Circulating Nurse.

• Ensures that Doctor’s orders are promptly. diligently, and properly carried out as specified in the patient’s chart.

• Records accurately in the chart the assessment and interventions done throughout the surgical or delivery procedures.

• Communicates /updates Attending Physician and other members of the surgical team regarding patient care rendered, status and other observations at least every 2 hours or when it is necessary.

• Observes medical/surgical asepsis at all times.

• Assist patients in moving, maintaining proper alignment and utilizing body mechanics at all times.

• Gives health teachings to patient and family during admission and before discharge of the patient.

• Monitors supplies, machine and articles used in every procedure and ensures its availability for the next patient’s OR procedure.

• Conducts monthly inventory of OR medicines, supplies, materials and equipment.

• Observes the prescribed OR/DR required uniform(clean scrub suit, OR cap, mask, and OR slippers) and maintains sterility of the work area.

• Inspects and keeps work area orderly and in good sanitary conditions at all times.

• Consults superiors on the operation or handling hospital property especially when in doubt of its use.

• Reports all property loss or damage to superiors or to proper authorities within 24 hours. JUNE 20,2014-Present

DUTIES AND RESPONSIBILITIES:

• Adequate medical assessment and history is taken in collaboration with other members of the healthcare providers such as the dietician or the physician

• Day-to-day patient interaction

• Vital signs checking (changes and alterations documented)

• Administration of medications and treatments (as pre ordered by physician)

• Changes dressings, bandages and contraptions as prescribed

• Diet supervision and monitoring (with considerations to dietician advise and medical condition)

• Providing or assisting patient in self-care
