Job Duties includes direct contact services (e.g., psychological, substance abuse, etc.) to inmates assigned to a specific living unit and caseload. Prepares initial social summaries (e.g. interviews inmates, reviews, records and compiles social summary reports as needed or assigned), and makes appropriate referrals to treatment units within the institution. The social worker also coordinates activities with other treatment units, volunteers and/or community service providers. Writes reports and correlates data as needed to include classification data, pre-parole summaries, admission summaries, parole risk assessments and security designation forms. Titled as a Mental Health Liaison, the social worker writes proposals for unit programs (e.g., treatment planning, case planning & risk assessment, utilizing Ohio Risk Assessment System). Develops, implements, and conducts unit programming for inmates on designated caseload.

May 2014 – Oct 2014 Position Summary: The Juvenile Forensic Therapist II (PT) functioned as a member of a team providing assessments for crisis screening services for youth in Montgomery County, who are or may be experiencing mental health and/or substance abuse problems or who may be victims of abuse, neglect have dependency issues. Services include crisis screening, lethality screening, consultation, educational and referral to other community resources. These services may be delivered by phone or face to face at the Montgomery County Juvenile Court Detention Center.

April 2009- Dec. 2009: DayMont Behavioral Health Care Inc.: Case Manager, Community Support Specialist

Under the supervision of the Manager of Community Psychiatric Supportive Treatment Services, I provided direct care and case management services to the Severely Mentally Disabled (SMD) population. My essential job functions and responsibilities were to: Assist SMD clients in meeting their housing needs; assist SMD clients in meeting their non-mental health, medical, dental and other health needs; assist SMD clients in obtaining all economic and other benefits to which they are entitled. The caseload assigned to me by the Unit Manager was an active caseload of 40-48 clients. I was required to maintain several direct care hours assigned by the agency. I was responsible for developing and maintaining ISP's on active cases, and must insure that all client documentation is appropriately completed with supervisor sign-off, as well as perform other duties and/or assignments as required by the agency.

2007-2008: Dayton Veterans Medical Center.

I performed social work duties under the supervision of a licensed social worker. My duties included working the intensive care, and acute care units. I performed assessments, placement, discharge planning. I also linked patients to aftercare facilities, and community links.
