Initially transported patient to ophthalmology where finding of papilledema necessitated an emergent MRI and subsequent lumbar puncture (LP) to determine intracranial pressure (ICP) level. Acquired physician instructions regarding treatment plan and interpreted to patient while patient was followed by Pediatric Neurology in Macon, GA and Neuro-ophthalmology in both Atlanta, GA and in Temple, TX. Accompanied patient to numerous LPs, MRIs and unsuccessful blood patch to treat a spinal headache. Assessed patient for acute signs of recurring IH: visual field changes/headaches/ other neurologic deficits. Assisted this patient in reordering medications, obtaining them from pharmacy, and preparing medication dispenser, ensured that patient took as prescribed: Topamax 50 mg PO (oral) BID

(for maintenance of IH and prevention of migraine headaches), Co-Q 10 enzyme 200 mg PO BID, Magnesium oxide 500 mg PO QD, oral contraceptives (all for migraine prophylaxis), and Maxalt 10 mg PO PRN x 2 in 24 hours (for migraine onset). Monitored for signs and symptoms of drug-related side effects, inadequate therapeutic treatment, patient non-compliance and recurrence of IH. Additionally, patient has been diagnosed with depression and anxiety, requiring Prozac 40 mg PO QD and Atarax 50 mg PO QPM. Accompanied patient to mental health care appointments. It also determined that the patient did not have an arteriovenous malformation (AVM) or other space-occupying lesion and the final diagnosis was an “immature hypothalamus”. Instructed patient to consume prescribed daily 500-1000 cc Gatorade to increase her sodium intake to compensate for the syncope. Patient also diagnosed at age 10 years with Ulcerative Colitis, having acute onset -- colonoscopy revealed pan colitis. Accompanied patient throughout ICU admission that required a blood transfusion (raising Hemoglobin from 8 to 10)/complete bowel rest x 7 days/ trans-parenteral nutrition (TPN) via a PICC (peripheral percutaneous central) line/ intravenous (IV) steroids and plethora of PO medications. Assisted patient with reordering medications and obtaining them from pharmacy: Methotrexate 25 mg PO every week. Since medication has narrow window of between therapeutic and toxic, monitored closely for side effects in addition to recurrence of any non-acute bowel inflammation (flares). Reviewed charts of all patients admitted to dedicated Stroke Unit, focusing on the progress note written by the Attending Physician or NP. Reviewed the results of the laboratory tests from the previous day to determine ones needed presently -- these would include, but were not limited to, complete blood count (CBC)/chemistry panel (Chem)/Lipid panel/fasting blood glucose (FBG) and coagulation studies (as needed). Reviewed any results from diagnostic studies that had been reported. Depending upon the findings of the lab tests and diagnostic studies, would evaluate and follow-up with pertinent collaboration with Neurology and/or the Chief Resident to problem solve and seek options to treatment. Proceeded to the patient to perform an abbreviated physical examination. This exam included a neurologic exam that evaluated the cranial nerves (CNS) and the motor and sensory function of the peripheral nervous system

(PNS). Depending upon the findings of this exam, may have needed to consult the Neurologist on-call to obtain emergency diagnostic testing such as CT/MRI/Doppler studies and referrals to other specialists. Usually, the findings of the exam were within the expected course of recovery of a cerebral vascular accident (CVA) and no further work-ups or consultations were required. Electrocardiograms (EKGs) were obtained and routinely followed and conditions such as atrial fibrillation were treated accordingly. Blood pressure readings were monitored (via manual cuff – not via arterial lines) as well and treated within protocol guidelines. Monitored any IV Heparin administrations and maintained close communication with the nursing staff to provide oversight and input for all patients on the Stroke Unit. Provided leadership, education, and patient/ caregiver satisfaction while improving the quality of care for stroke victims. Wore a pager for notification of newly admitted stroke patients to the Emergency Room (ER). While the ER staff provided initial evaluation and treatment of suspected stroke as per hospital protocol, obtained an in-depth history from both the patient and their caregiver (if possible) and performed a mental status exam and extensive neurologic exam, including both the CNS/PNS. Wrote admission orders per stroke unit protocol, dependent upon the results of the initial diagnostic studies which involved complex clinical decision making dependent upon whether the CVA was ischemic or hemorrhagic. Any patient receiving tPa (tissue plasminogen activator) was automatically admitted to ICU for close monitoring. This was also the course of admission for any patient who was a candidate for surgical procedures, emergent or otherwise, and for any patient with massive stroke (determined via a score on a tool created by the National Institutes of Health). Those patients suffering a massive CVA required advanced directives (which, regarding, I educated the patient -- when possible -- and/or family members) and obtained, for the patient to be admitted to the Stroke Unit. All care provided during shift of duty required organizational prioritization dependent upon acuity of patient population/need. Provided psychological and social support to the patient and family by actively listening to any concerns and referring to social services for extenuating circumstances, particularly any concerns regarding community services and insurance benefits. Educated patient, caregivers, and nursing staff about the projected management of care and available resources and healthcare options. When appropriate, discharge planning was addressed and included transfer to the neighboring Stroke Rehabilitation Unit to individualize the treatment plan for patient needs. (tertiary level) Outpatient Same Day Surgery Clinic. Since numerous patients had co-morbid diseases, a thorough review of systems was crucial to intake and screen information on all of surgical candidate’s acute and chronic illnesses. Collaborated with both the primary care provider (PCP) and any appropriate specialty physicians to obtain patient medical and psychological history . Obtained and provided pertinent information via phone consultation. Responsible for obtaining laboratory and diagnostic studies -- CBC/serum creatinine and electrolytes/blood glucose/ECG/chest x- ray (CXR)/coagulation studies most common but not exclusive. Evaluated findings to determine if candidate was ‘safe’ for anesthesia and procedure. Dependent upon the particular co-morbid disease, other tests could require monitoring -- most specifically, therapeutic levels of maintenance medications. Performed system-specific physical exam diagnostics, for example, doppler pulses for perfusion evaluation. Wrote pre-op orders as per anesthesia and surgeon protocol -- for antibiotics pre and post-operatively, IV fluid pre and post-op, resuming therapeutic medications for chronic illnesses (oral hypoglycemics/insulins; inhalers/bronchodilators etc.) Additionally, pre-op education included patient and caregiver instruction regarding the following: basic details of the surgical procedure; type of anesthesia administered; typical post-op expectations; potential post-op complications; post-op signs and symptoms requiring notification of PCP; medications to avoid 7-10 days prior to OR (specifically anticoagulants/NSAIDS/ aspirin/herbal supplements such as willow bark). Provided direction and guidance for follow-up appointment date with the surgeon. Provided instruction regarding post- operative pain management. Role as a nurse practitioner in the Cerebral Palsy Clinic at Riley Children’s Hospital was that as a specialty clinician, not as a primary health care provider. Assessed that child with CP was in a state of wellness, and not acutely ill. Goal of the annual visit to center was for planning interventions, authorizing treatments, follow-up on condition and social needs and support structure of family/caregivers. Optimizing health and well-being for children and caregivers was family-centered with co- management as the model. Diagnostic assessment and management of children with CP is classified into the following categories: motor abnormalities

(type/function/musculoskeletal changes); associated impairments (CNS/GI/ Respiratory/ Genitourinary/ Dental); distribution (anatomic – limbs/radiologic periventricular leukomalacia and damage to basal ganglia or thalamus); causation and timing; and severity. Scope of practice emerged from these categories. Obtained an accurate review of systems for tracking medical conditions through extensive history and physical exam of the child. Medical conditions addressed at each visit included: growth, nutrition, pain, sleep, drooling, swallowing, respiratory infections, aspirations, and restrictive lung disease, constipation, reflux, toilet training, voiding dysfunction, urinary infections, gynecology and puberty (often precocious), joint contractures/ dislocations, scoliosis, osteoporosis, orthotic devices, seizures, muscle tone/strength or weakness, attention(deficit or hyperactivity), pressure sores, communication (speech therapy), assistive devices, scoring on cognitive testing, and educational placement. Followed the American Academy of Neurology guidelines for pharmacologic treatment of spasticity in children with CP -- recommended treatments included injections of botulinum toxin into spastic muscles as well as intrathecal baclofen. The guidelines also included systemic medications including diazepam, baclofen, and dantrolene. Assessed the family by listening carefully to parental concerns and offered referrals to community resources including parent support and advocacy groups/respite programs in an attempt to alleviate any stress and to enhance parental well- being.
