JOB DESCRIPTION

HEMODIALYSIS STAFF NURSE

This position is responsible for the delivery of all dialysis related nursing services for the End Stage Renal Disease patient in a chronic setting.

Supervises and performs direct patient care.

Notes signs of trauma, patients’ general physical and mental condition, reviews dialysis orders, documents observations in patient medical records.

Communicates all emergencies, patients’ life threatening complications and related incidents.

Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient’s state of health and well- being.

Provides dialysis therapy, including but not limited to formulating dialysis plan per physician orders, weight and rate to be ultra-filtered, ensuring safety measures are in place and complied with, administering medications or other appropriate treatments, discontinuing dialysis according to established procedure, documenting all activity appropriately.

Administer patient quality care in a considerate, respectful manner. COORDINATOR NURSE OF INTEGRATION

This position is responsible for quality and effectiveness of the Integration process, to allow a smooth openings of the centers with all the processes and resources in place. Helps on the startup of operations, contributing for healthy environment on the cost controls and teammate's relations to ensure effective, efficient and safe operations in accordance with DaVita Healthcare Policies, Procedures and Guidelines.

Provides support, guiding, mentoring, coaching and teaching teammates during first days (1 to 3 months according to the situation).

Consolidate the knowledge and continuous improvement, service excellence on integration

Supervises the nurses in difficult cannulation access of the patients.

Works with head nurse, charge nurse and staff nurses hand in hand during the initiation phase to have smooth opening.
