Clinical Staff Nurse-Heart failure Nurse Navigator March 2014- present
Developed and implemented the nurse navigator role for the heart failure patient. Responsibilities include education/support of the heart failure patient across the continuum from inpatient to home incorporating self-management education, discharge planning, post-discharge telephonic follow-up, and post-discharge education/cardiac rehabilitation. Other roles include developing liaison relationships/providing education support between the hospital and post-discharge care providers.
Clinical Staff Nurse-Outpatient Cardiology 1994- 2014
Care of outpatient cardiac and pulmonary rehabilitation patients for phase II and phase III rehabilitation including telemetry monitoring/hemodynamics, admission/session/discharge charting, and patient/family education. Managerial duties include patient accounting and billing, departmental payroll/scheduling, and intern supervisor for exercise physiology students. Current experience in exercise and pharmacologic stress testing including stress echo, MPI, adenosine, and dobutamine.
